1. Pendahuluan
Thromboangiitis obliterans (TAO) adalah nonatherosclerotic, penyakit
radang segmental yang paling umum mempengaruhi arteri kecil dan
menengah dan vena pada ekstremitas atas dan bawah. Dalam
karakteristik fase akut lesi, berkaitan dengan trombosis seluler oklusif,
peradangan akut yang melibatkan seluruh lapisan dinding pembuluh
menyebabkan TAO harus diklasifikasikan sebagai vaskulitis a. TAO dapat
dibedakan dari jenis lain vaskulitis berdasarkan kecenderungan untuk
terjadi pada subjek laki-laki muda, hubungannya yang erat dengan
konsumsi tembakau, kelangkaan tanda-tanda sistemik dan gejala, trombus
yang sangat selular dengan hemat relatif dinding pembuluh darah, dan

Thromboangiitis obliterans (TAO) pertama kali dijelaskan pada 1879,
ketika Felix von Winiwarter, seorang ahli bedah Austria yang merupakan
asosiasi dari Theodor Billroth, melaporkan dalam Archives of Surgery

2. Epidemiologi
Prevalensi penyakit di antara semua pasien dengan eral rentang penyakit
arteri peripher dari nilai-nilai serendah 0,5 ke 5,6% di Eropa Barat dengan
nilai-nilai setinggi 45-63% di India,16-66% di Korea dan Jepang, dan 80%
di antara orang-orang Yahudi Ashkenazi keturunan yang tinggal di Israel
[3]. TAO awalnya diduga mempengaruhi hampir secara eksklusif laki-laki,
karena kurang dari 1% dari mereka yang terkena dampak adalah
perempuan. Dalam studi terbaru, proporsi pasien TAO wanita bervariasi
antara 11% dan 23% [4]. Peningkatan ini mungkin karena peningkatan
merokok di kalangan wanita.
3. Etiologi dan Patogenesis
5. Fitur Klinis
Presentasi klasik penyakit Buerger adalah seorang perokok laki-laki muda
dengan timbulnya gejala sebelum usia 40 sampai 45 tahun. TAO
bermanifestasi sebagai tromboflebitis migrasi atau tanda-tanda
insufisiensi arteri pada ekstremitas. Dalam sebuah studi oleh Sasaki et al.
[26] mata pelajaran yang 749 pria dan 76 wanita, dengan usia rata-rata
50,8 0,4 tahun. Pada 42 pasien (5,1%) keterlibatan terbatas pada arteri
ekstremitas atas, di 616 pasien (74,7%) penyakit terbatas pada tungkai
Iskemia tungkai atas secara klinis terbukti dalam 40-50% pasien, tetapi
dapat dideteksi pada 63% pasien dengan Allen test [29] dan di 91% dari
pasien dengan arteriogram tangan dan lengan [30]. Dalam tes Allen,

arteri dari satu tangan pasien. Pasien membuka kepalan tangan dan
pemeriksa kemudian melepaskan tekanan dari arteri radial tetapi tidak
arteri ulnaris. Jika distal arteri radial pada pergelangan tangan adalah
paten, ada yang cepat kembali ke warna pada tangan (tes negatif). Jika
arteri tersumbat, tangan akan tetap pucat (tes positif). Manuver diulang
dengan tekanan dilepaskan dari arteri ulnaris tetapi tidak arteri radial.
5.2. Tromboflebitis superfisial. thrombophleBitis superfisial diamati dalam
40-60% kasus. Vena dalam flebitis tromboemboli tidak biasa dan sugestif
dari diagnosis alternatif, seperti penyakit Behcet. Tromboflebitis superfisial
ini bermigrasi dan berulang dan mempengaruhi lengan dan kaki. Migrasi
flebitis (saltans flebitis) pada pasien muda karena itu sangat sugestif TAO
[31].
5.3. Tanda dan Gejala sistemik. Tanda-tanda sistemik dan gejala sangat
jarang pada pasien dengan TAO. Telah ada laporan sesekali keterlibatan
pembuluh visceral. Iskemia pencernaan dapat bermanifestasi sebagai
sakit perut, diare, penurunan berat badan, atau melena. Perforasi usus
dan mesenterika infark dapat terjadi. Ada laporan dari TAO awalnya
menyajikan iskemia usus kecil dan obstruksi kolon [32, 33].
Dalam beberapa kasus, kerusakan arteri viseral hasil lebih mungkin dari
aterosklerosis, disukai oleh atau berhubungan dengan TAO. Jadi, ketika
TAO terjadi di lokasi yang tidak biasa, diagnosis harus dibuat hanya
setelah identifikasi lesi vaskular khas inflamasi pada pemeriksaan
histopatologi.
Keterlibatan sistem saraf pusat telah dilaporkan di TAO yang dapat dilihat
sebagai serangan iskemik transien atau stroke iskemik. Pemeriksaan
histologis postmortem telah menunjukkan peradangan arteri kecil dan
menengah dari leptomeninges atau bahkan dari meninges atau vena [34].
8. Pengobatan
Pengobatan yang paling efektif untuk penyakit Buerger adalah berhenti
merokok. Segala cara yang mungkin harus digunakan untuk mendorong
pasien untuk menyerah penggunaan tembakau, dalam segala bentuknya.
Pendidikan pasien adalah penting, tetapi hanya 43-70% kasus berhasil
berhenti merokok [39]. Bantuan psikologis mungkin berguna dalam kasuskasus tertentu, tetapi pasien harus diyakinkan bahwa jika mereka berhasil
berhenti merokok sepenuhnya, penyakit akan masuk ke remisi dan
amputasi bisa dihindari. Reseptor cannabinoid selektif antagonis, seperti
rimonabant, telah menunjukkan hasil yang baik dalam membantu pasien
berhenti merokok [39].
Penelitian
obliterans
thromboangiitis
Eropa
(penyakit
Buerger)
menunjukkan tidak ada perbedaan signifikan yang ditemukan antara
kelompok untuk penyembuhan total lesi [57]. Mohler dkk. melaporkan
secara acak, percobaan lain tersamar ganda, dikontrol menggunakan
beraprost natrium, sebuah prostaglandin I2 analog secara lisan aktif untuk
untuk sel induk BM. Dalam sebuah studi oleh Moriya dkk. [66] pada 42
pasien dari TAO diobati dengan darah perifer sel mononuklear induk,
perbaikan gejala iskemik diamati pada 60% sampai 70% dari pasien.
Tingkat tahunan amputasi mayor menurun tajam dengan pengobatan.
Dalam sebuah studi oleh Kawamoto dkk. [67] di 17 pasien dengan TAO
yang diobati dengan injeksi intramuskular G CSF dimobilisasi CD34 + sel
dari darah perifer. Pengamatan Selama 12 minggu setelah terapi sel,
angka Wong-Baker FACES skala nyeri, tekanan oksigen parsial transkutan,
total atau bebas rasa sakit berjalan kaki, dan ukuran ulkus serial membaik
pada semua pasien.
Terapi sel menggunakan sel sumsum tulang mononuklear (BM-MNC) dan
sel mononuklear darah perifer (PBM- NCS) memiliki hasil yang efektif pada
pasien dengan penyakit arteri perifer dan TAO. Jaringan adiposa berlimpah
dalam tubuh manusia dan secara konsisten diisi ulang. Oleh karena itu,
jaringan ini merupakan sumber ideal MSC. Telah terbukti bahwa jaringan
adiposa berasal MSC (ATMSCs) memiliki karakteristik mirip dengan sel-sel
sumsum tulang stroma (BMSCs) [68]. ATMSCs berdiferensiasi menjadi selsel endotel dan memiliki efek proangiogenic. Dalam sebuah studi oleh Lee
et al. [69] 15 pasien dengan TAO diobati dengan suntikan intramuskular
ATMSCs; terlihat bahwa perbaikan klinis terjadi di 66,7% pasien. Lima
pasien diperlukan amputasi minor selama ikutan, dan semua situs
amputasi sembuh sepenuhnya. Pada 6 bulan, peningkatan yang signifikan
tercatat pada skala penilaian nyeri dan klaudikasio jarak berjalan kaki.
Angiografi pengurangan digital sebelum dan 6 bulan setelah implantasi
ATM- SCS menunjukkan pembentukan berbagai jaringan agunan vaskular
di arteri terpengaruh. Skala besar lebih lanjut percobaan acak diperlukan
untuk menilai manfaat jangka panjang dari terapi sel induk.
9. Kesimpulan
TAO adalah bentuk yang berbeda dari vaskulitis sistemik etiologi tidak
diketahui meskipun sangat terkait dengan merokok. Gambaran klinis dan
angiografi membentuk dasar utama diagnosis. Pantang dari merokok
adalah satu-satunya pengobatan definitif untuk mencegah perkembangan
penyakit. Garis medis pengobatan dengan vasodilator, pentoxyfylline, dan
cilostazol dapat membantu meningkatkan bebas rasa sakit berjalan kaki
tetapi tidak dapat mencegah penyakit pro- gression. Pengobatan bedah
berupa
revaskularisasi,
simpatektomi,
Ilizarov,
dan
meningkat
omentopexy periph aliran darah eral dan mengurangi tingkat amputasi.
Terapi baru dengan prostaglandin, bosentan, dan terapi sel induk telah
menunjukkan hasil yang menjanjikan. Diagnosis dini dan terapi agresif
dapat menurunkan gejala pasien dan kemungkinan amputasi utama.
