Dear Atty. Agustin:
Greetings!
I, the undersigned, is humbly request to grant my prayer to allow me to report
for duty with my current plantilla position as Teacher III at San Mateo
Vocational and Industrial High School in Brgy. San Andres, San Mateo, Isabela
anytime upon notice.
To give you a background on the circumstances of my approved leave of absence,
sometime in June 2016, I personally filed a sick leave without pay at San Mateo
Vocational and Industrial High School-San Mateo, Isabela where I was employed
as permanent employee with a plantilla of Teacher III for the duration of six (6)
months covering the period of June 17, 2016 to December 17, 2016 following of
my immediate voluntary surrender to the Philippine National Police San Mateo,
Isabela by virtue of Warrant of Arrest issued by presiding Judge Raymundo L
Aumentado of RTC Branch 20, Cauayan City, Isabela for violation of anti-rape
law and later, I was committed at BJMP San Mateo, Isabela while waiting for the
trial of my case.
However, with my prayer to dismiss my criminal case because of lack of interest
on the part of the complainant to prosecute the case, on September 14, 2016, with
the Honorable Court of Branch 20, Cauayan City presided by Judge Aumentado
issued an Order for the provisional dismissal of the case for insufficiency of
evidence brought about the lack interest on the part of private complainant and
her mother to prosecute me the information filed against me with Criminal Case
Number 20-10665 for the case of Rape hereto attached.
Hence, I, the undersigned, request to report at San Mateo Vocational and
Industrial High School in Brgy. San Andres, San Mateo, Isabela for duty upon
notice.
