CHAPTER I

INTRODUCTION
1.1 Background
The field of endocrinology encompasses the study of glands and the
hormones they produce. Unlike many other specialties in medicine, it is not
possible to define endocrinology strictly along anatomic lines. The classic
endocrine glands are pituitary, thyroid, parathyroid, pancreatic islets,
adrenals, and gonads, communicate broadly with other organs through the
nervous system, hormones, cytokines, and growth factors. However, the
clinical specialty of endocrinology is most clearly delineated by diseases that
afflict

the

classic

glands.

Hypothyroidism,

hyperthyroidism,

insulin

resistance, and adrenal insufficiency are some of the examples. In various

parts of the world, additional clinical disorders, such as hypertension,
nutrition, obesity, osteoporosis, and hyperlipidemia, also fall within the scope
of endocrinology.
Nowadays, some diseases that afflict endocrine system become
worldwide trend since their prevalence are increasing year by year, for
example is diabetes mellitus. Right now, diabetes mellitus becomes global
pandemic for its prevalence is increasing every year. The worldwide
prevalence of diabetes mellitus has risen dramatically over the past two
decades, from an estimated 30 million cases in 1985 to 382 million in 2013.
Based on current trends, the International Diabetes Federation projects that
592 million individuals will have diabetes by the year 2035. Although the
prevalence of both type 1 and type 2 diabetes mellitus is increasing
worldwide, the prevalence of type 2 diabetes is rising much more rapidly,
presumably because of increasing obesity, reduced activity levels as countries
become more industrialized, and the aging of the global population.
Diabetes mellitus, both type 1 and 2, if not treated properly, will cause
the patients to fall in a condition of acute hyperglycemia or hypoglycemia.
Diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar state (HHS)
are the most serious hyperglycemic emergencies in patients with diabetes.

The overall mortality in children and adult subjects with DKA is less than 1%,
but a mortality rate higher than 5% is reported in older adults and in patients
with concomitant life-threatening illnesses. Mortality in patients with HHS is
higher than in those with DKA and is reported in up to 20% in recent
observational studies. In the other hand, hypoglycemia is a complication that
most commonly caused by drugs used to treat diabetes mellitus or by
exposure to other drugs, including alcohol. An estimated 610% of people
with type 1 diabetes die as a result of hypoglycemia. The incidence of
hypoglycemia is lower in type 2 diabetes than in type 1. However, its
prevalence in insulin-requiring type 2 diabetes is surprisingly high, so the
prevalence of hypoglycemia is now greater in type 2. Not only insulin, but
sulfonylureas, or glinides also can cause hypoglycemia in type 2 diabetes.
Not only diabetes mellitus complications that need immediate
emergency treatment, but also other endocrine disorders. For examples are
myxoedema coma, thyroid storm, and adrenal crisis, all of them need
immediate treatment to reduce its morbidity and mortality. Therefore,
developing

knowledge

about

emergency

medicine

in

the

field

of

endocrinology is necessary. Other type of endocrine diseases, which are not

emergency, are also essential to be accounted as their presence deteriorates
patients quality of life, moreover such diseases are also potential to be the
precipitating factor of another more severe endocrine disorder. Some of the
examples are obesity, osteoporosis, hypertension, and hyperlipidemia. Hence,
it is necessary to find innovative and creative ideas to solve those problems.
Furthermore, students as the agents of change, have a great role in
developing science through their ideas, especially medical and health sciences
students who are able to improve the population health quality. Therefore,
we are the students of Faculty of Medicine, Universitas Brawijaya would like
to invite and ask all students in the world to create innovative and creative
ideas in the form of literature review, scientific abstract, and health campaign
through Medical Fiesta 2016 program. Our theme is Endocrine Diseases and
will be ended with International Seminar on Emergency in Endocrine Disease.

1.2 Objectives

PROGRAM DESCRIPTION
2.1 Title of the Program
The program is titled Medical Fiesta.
2.2 Theme of the Program
The program is under the theme of Holistic Approach to Endocrine Diseases.
2.3 Target and General Activities of the Program
2.3.1 Target of the Program
The target of the program is medical and health science
undergraduate students, both from Indonesia and outside Indonesia,
within Southeast Asia. The competition and symposium is within
international level.
2.3.2 General Activities of the Program
Medical Fiesta is an annual official program under semiautonomous student organization in Faculty of Medicine, Universitas
Brawijaya, i.e. Lembaga Studi Ilmiah Mahasiswa (LSIM) or Medical
Students Research Organization. The following are the series of event
in Medical Fiesta 2016.
Research Abstract and Paper Competition (RA)
RA is a competition in writing research abstract between
medical and health science undergraduate students. The research
abstract is an original research competition for all fields of medical and
health science research. The research model could be original report,
case report, or epidemiological studies. From all abstracts that had
been published, there will be evaluation to select 10 best research
abstract that will run the final round for full paper oral presentation to
decide the three winners and one best abstract.
Literature Review Competition (LV)
LV is a competition in synthesizing literature review between
medical and health science undergraduate students within the theme
of Holistic Approach to Endocrine Diseases. From all the submitted

reviews, there will be evaluation to select 10 best literature reviews that

will run the final round for oral presentation to decide the three
winners.
Public Campaign Competition (PC)
PC is a competition in making creative public poster and
videography between medical and health science undergraduate
students within the theme of Holistic Approach to Endocrine Diseases.
From all the submitted videography and posters, there will be
evaluation to select 9 best campaigns that will run the final round for
oral poster presentation to decide the three winners. Besides, from the
online voting that had been conducted previously, there will be
assessment to decide one favorite public campaign.
Hands-on Workshop
This hands-on workshop is an event that consists of clinical
skills materials encompasses the management of Diabetes Mellitus
under the theme of Enhancing Skills in Managing Diabetes Mellitus. The
workshop will be tutored by staffs from Internal Medicine and
Nutrition Department, encompasses Screening and Diagnosis of
Diabetes Mellitus Complications, Oral Anti Diabetes, Insulin, and Food
Modelling through discussion, tutorial and hands-on simulation in a
small scale group that will be rotated in a certain duration.
International Seminar
The international seminar is a closing event of Medical Fiesta
2016. This year, the theme of the seminar is Clinical Updates on
Emergency Endocrinology: Optimizing Management of Endocrine Diseases
Fatal Complications. The event invites all finalists of Medical Fiesta 2016,
students, and health practitioners with the expert speakers from
Indonesia, Thailand, and Malaysia.
