JOINT AFFIDAVIT
WE, ISMAEL FRONGOSO-GUAZON, widower, 84 years old, and
ROGELIO H. HERNANDEZ, widower, 72 years old, residents of Sto.
Domingo, Nabua, Camarines Sur, after having been sworn to in accordance
with law, hereby depose and state that:
1. That we both personally know the persons of the spouses Jose Ornido and
Rosario Payawat both residents of Sto. Domingo, Nabua, Camarines Sur,
Philippines;
2. That said ORNIDO spouses had eight children and one of whom is their
seventh child, Manuel Payawal Ornido, born on June 18, 1960 at Sto.
Domingo, Nabua, Camarines Sur;
3. That his father and mother died on April 26, 1992 at Magallanes, Iraya,
Daet, Camarines Norte and November 18, 2009 at Magallanes, Iraya, Daet,
Camarines Norte, respectively;
4. That we know the foregoing facts because we are close friends and
acquaintances of the ORNIDO spouses and thus privy to any or all family
events;
5. That we are not parties-in-interest nor we related by affinity nor
consanguinity to said ORNIDO spouses;
6. That we are executing this affidavit to attest to the veracity of the
foregoing contents giving the authorities concerned and the correct data,
information and reference for the purpose of attesting the present
whereabouts of the affiants mother as one of the requirement for the
delayed registration of the affiants birth and for whatever legal purpose this
may proper, wise and equitable under the premises.
FURTHER AFFIANTS SAYETH NAUGHT.
