AFFIDAVIT OF WAIVER OF RIGHTS

I,DANVIE S. RANAY, of legal age, married, Filipino
citizen and a resident of Brgy. 10, Estancia, Pasuquin,
Ilocos Norte, Philippines after having been sworn to in
accordance with law hereby depose and say that:
That I engaged the services of ENGR. AMOR C. CRUZ, a
Professional Electrical Engineer who signed the electrical
permit form of my Application for Electrical Permit and
WILFREDO GORICHO who is in-charge of electrical installation
of my application of electrical permit form;
That however, ENGR. CRUZ is nowhere to be found
and
cannot sign the Occupancy and asbuilt plan form, and
Wilfredo Goricho who did not supervise the electrical
installation;
That as a result thereto, I engaged the services of
another Professional Electrical Engineer by the name of
ALEXANDER R. RAMONES and DESCARTES F. JUSTO, a registered
Master Electrician, who is in charge of installation to take
the place of ENGR. AMOR C. CRUZ and WILFREDO GORICHO, in
signing
my
Certificate
of
Final
Electrical
Inspection/Completion and as-built plan of my residential
building located at Brgy. 23, Dungtal, Laoag City, Ilocos
Norte;
That I am executing this affidavit to attest to the
truth of all the foregoing and for all legal intents and
purposes it may serve;
