Invoice must be paid in full no later then December 15, 2016.
Credit Card Policy: Fill in credit card information legibly. Form must be signed and dated by the card holder for validation. For verification purposes, address
information must be filled out as it appears on the credit card monthly statement. If address is different then company contact information please include in
payment area. Failure to complete the address information, including zip code, may result in the payment not being accepted by your credit card institution.
Termination
You or the Association may terminate this Agreement upon material cause by providing thirty (30) calendar days written notice. However, if you cancel after the
Association has begun production of materials listing your name, then you are responsible for payment. Upon termination or expiration, no further use
may be made of the Association Marks, or other proprietary property or materials provided, developed or intended for use in connection with the sponsorship,
without prior written authorization.
Indemnification and Insurance
Company and the Association agree to indemnify, defend and hold harmless each other, their officers, directors, employees, volunteers, subcontractors and
agents, from any and all claims, losses, damages, liabilities, judgments, or settlements, including reasonable attorneys' fees, costs, and other expenses incurred
on account of the indemnifying partys negligent acts or omissions in connection with the Sponsorship. In addition, Company shall maintain insurance of not
less than $1,000,000 for comprehensive general liability, with evidence of such insurance provided to the Association within 30 days of the signing of this Agreement.
