Greetings of peace!
Consistent with its mission of providing quality education, the universitys curriculum in Bachelor
of Science in Office Administration major in Legal Transcription requires our graduating students to
complete 100 hours of on-the-job training in legal procedures. This exposure and training would not
only make them be acquainted with legal proceedings and familiarized with legal terminologies but
also practice their skills in stenography.
In this regard, we would like to request your humble office to accommodate Mr. Julius Eric T.
Alcantara on-the-job trainee. He is required to accomplish the above-mentioned number of
accumulated training hours before the final examination this semester. May we further request for
her to be allowed to observe/attend court hearings during her stay in the office. Please feel free to
arrange her work schedule. He is available during Wednesdays and Fridays.
If accommodated, the trainee will be required to submit the company profile, make a journal of
daily activities, and share office experiences for classroom discussion purposes. We will also
request her immediate supervisor to rate the trainee which will be part of the trainees final rating on
the subject.
Any assistance you could offer to our student shall be given utmost appreciation as it makes her be
fully equipped with gainful knowledge and skills, hence, more prepared as He gets employed in her
chosen field of specialization. We are hoping that you share our concern of making education more
relevant through exposure in the real world.

WAIVER/UNDERTAKING

(parent/guardian), hereby execute this Waiver/Undertaking to hold the Polytechnic University of the
Philippines and the training institution free from any liability for any injury / illness / loss of life or
property that may be incurred / sustained by the student-trainee in the course of her on-the-job
training / practicum with the training institution provided that the same is not due to any willful
criminal act or gross negligence of the training institution or any of its representatives, officers, or
employees.
As the parent/guardian of the student-trainee, I hold myself responsible for any expenses that may
be incurred as a result of the students illness, injury, death or loss or property in the course of the
students on-the-job training/practicum, except as provided above. I further undertake to be
responsible for any breakage, loss of property, damage, death or injury that may be caused by the
student-trainee to any party while undergoing training.
We execute this Waiver/Undertaking in accordance with our free will, realizing the benefits that the
student-trainee shall obtain from the said on-the-job training/practicum.
