AFFIDAVIT OF LOSS
I, EMILY GEPTE BANQUIL, Filipino, of legal age, married and a
resident of 25 Lawaan Street, Doa Pilar Subdivision, Sasa, Davao City,
after having been duly sworn to in accordance with law, do hereby depose
and say:
1. That I am the same and identical person who caused the execution
of this instrument;
2. That I am a sister of NOEMI GEPTE KATSUMATA. Our mother is
ANTONIA TIMUAY SAPI and our father was the late EMMANUEL
GARCIA GEPTE. To prove this fact, I attached hereto as ANNEX
A my CERTICATE OF LIVE BIRTH and that of NOEMI (maiden
name NOEMI SAPI GEPTE) as ANNEX B. Both Certificates of
Live Birth indicate that we have the same parents;
3. That our parents were married on MAY 15, 1967 at COTABATO
CITY. Attached hereto as ANNEX C is their MARRIAGE
CONTRACT;
4. That our parents borne seven (7) children. I am the fifth child and
NOEMI is the sixth in order of birth. Aside from the two us, the
following are also our siblings with their order of birth: LEMUEL
SAPI GEPTE (1st child), HELEN SAPI GEPTE (2nd child), VELINDA
SAPI GEPTE 3rd child), EMMANUEL SAPI GEPTE JR. (4th child) and
JEANILYN SAPI GEPTE (7th and last child). Attached hereto are
their CERTIFICATES OF LIVE BIRTH respectively as ANNEX D
to ANNEX H;
5. That I am executing this affidavit to prove that I and NOEMI GEPTE
KATUSUMATA are siblings and for whatever legal intents and
purposes this may serve;
6. That I am executing this affidavit freely and voluntarily.
IN WITNESS WHEREOF, I have hereunto set forth my hand this 3 rd
