Diuretics are given regularly in the acute care setting.

Which of the
following clients is not a good candidate for diuretics?
Client with pulmonary edema
Client with hypercalcemia
Client with hypokalemia
Client with kidney stone formation
The client with hypokalemia is not an ideal candidate for a diuretic because
diuretics not only rid the body of excess fluid but they take electrolytes with it.
If the patient is already dealing with low potassium, a diuretic will worsen the
level and put the patient at increased risk of harmful dysrthymias.
True or False
All health care professionals are to be competent in fire safety and should
know where the oxygen turn-off valve and exits are on the acute care
setting they work.
True. This is important because anything can happen staring with malfunction
or an emergency such as a fire. Oxygen can cause the fire to intensify so all
staff should know how to handle oxygen just in case but only after placing
patients in a safe place.
A client with thyroid cancer is receiving brachytherapy. Define this
therapy.
Brachytherapy is a kind of radiation therapy that is placed internally to irradiate
the cancer and destroy it at the target area. The patient is placed in a private
room and the nurse must wear a dosimeter badge to keep up with patient
contact/interaction time to limit exposure. The radiation can be delivered by
radioactive seeds, external beam, or by IV.

A nurse is providing education on home safety specific to the school-age

client to parents. Identify four (4) examples of home safety concerns for
this age group.
The school age range form 6 to 12 years of old. The school age child is
becoming more aware of themselves and their parents have a big influence on
them. At this age, they are still in danger of several things but are more
confident through their abilities. At home, they should become aware and stay
safe while playing and should be well aware of stranger danger. They should
wear their helmets, knee and elbow pads when on skates and bicycles. This age
loves to cook with mom, so they should be aware of the dangers of cooking and
how to avoid burns and fire. They should wear sunscreen when at the pool but
also be taught how to swim and how to avoid drowning and just other
important water safety measures. They should always ride in the back seat and
avoid injury from airbags. Always wear their seatbelt and parents should be
their example. When it comes to cleaning the house, they should be taught how
to use the products and how to store them. Some parents may own firearms,
and in this case, they should be taught the dangers of them and never to touch
the firearms.
A newborn client has developed hyperbilirubinemia. What is the
treatment for this newborn complication?
The infants bili level must be monitored the entire time until normal
Assess the newborns blood type for compatibility of ABO
Do blood work to assess Hgb and Hct
Do a direct Coombs test for the Rh positive RBC
Check hydration status during phototherapy and be sure to provide fluids.
Monitor elimination and daily weight.
Provide feedings Q3h to promote excretion of bili
Perform vitals, especially temperature during therapy.
A transcutaneous bili test should be performed to measure the bili level
Treatment is phototherapy which includes placing an eye mask to protect
corneas and retinas, the baby will have on a surgical mask to cover the
genitalia. No lotions or ointments on skin due to the absorption of heat that can
cause burning. Reposition the infant Q2h to be sure all body parts are exposed
to the therapy.
Turn off therapy during blood draws and to have bonding time with parents
Be sure to remove the infant Q4h to assess for injury or inflammation
Bronze discoloration is not a concern nor is maculopapular skin rash
Explain to parents that hyperbilirubinemia happens to most newborns
Explain the color of the bile is green and will be loose.
When a infants bili level >15mg/dL, they will be prescribed bili therapy.

What are the expected findings associated with a pediculosis capitis

infestation?

This is head lice which is spread by direct contact from person to person,
bedding and clothing. The lice lay eggs at night and close to the skins surface at
the hair shaft. There is intense itching, small red bumps on the scalp, white
specks from the nits at the hair shaft. To rid of the nits, use a permethrin
shampoo with a special comb that is made to remove nits. Then repeat
application of shampoo in 7 days. Wash bedding in the hottest water possible.
True or False
The restraint prescription must include the reason for the restraint, the
type of restraint, the location of the restraint, how long the restraint may
be used, and the type of behaviors demonstrated by the client that warrant
use of the restraint. True. Proper documentation is crucial when it comes to
restraints and why. Also, when a restrain is in place, the nurse must assess for
skin integrity hourly. The restraints must be removed per order and a new order
must be in place every 24 hrs.
