AFFIDAVIT OF FAMILY COMPOSITION

I, Jessica Faustino Canillas, single, of legal age, Filipino, with residence and
postal address at No. 2426 Tramo Street, Buendia, Pasay City, after having been sworn
to in accordance with the law, do hereby depose and say:
1. That my parents are Virgilio Palabrica Canillas and Samuelita Atamosa
Faustino who were married alst July 30, 1972 in Toril, Davao City;
2. That I was born on July 31, 1975 in Tagum City, Davao as evidence of
herein attached Certificate of Live Birth issued by the National Statistics
Office;
3. That after my birth, my father, Virgilio Canillas, abandoned us and my
mother and I lived alone until I reached the age of twenty (20). To date I
have no knowledge of hiswhereabouts nor his present status or condition
whether living or deceased;
4. That from twenty years onwards, I am already supporting myself and hav e
been living independently since my mother, Samuelita Faustino, moved in
and cohabited with another man and they have been moving from one
place to another. The last communication I have with her was on December
2009 and to date, no communication was made between us;
5. That I born an ONLY CHILD and have no other siblings ;
6. That I am executing this affidavit to attest the truth of the foregoing facts
without intent to circumvent any of the existing laws and regulations.

3. That I born an ONLY CHILD and have no other siblings ;
4. That on April 12, 2005, I contracted marriage with one Hiromitsu
Yoshinari which was celebrated in Tokyo, Japan. Said marriage was later on
dissolved on March 27, 2015 per Divorce Certificate issued by the Embassy
of Japan and duly authenticated by the Department of Foreign Affairs and
registered with the Local Civil Registrar of Manila.
5. The Divorce Decree was Recognized and enforced through the decision
of the Regional Trial Court Branch 113, Pasay City, penned by Judge
Caridad H. Cuerdo, which capacitated me to re-marry;
6. That I am executing this affidavit to attest the truth of the foregoing
facts without intent to circumvent any of the existing laws and regulations.
