I. BACKGROUND

In addition to the field of economic development and education,

development of the health sector is also a very important part in the
Indonesian government seeks to prosper the country and lead to the
prosperity of his nation.

Currently the Indonesian public awareness of the importance of increasing

health, it has a direct impact on the utilization of health care units. But
unfortunately there are still many areas in Indonesia where people are
constrained to obtain adequate health care.

In meeting health care needs of the community as a whole, either through

government agencies and various related parties continue to be pursued, but
still there are many areas that the majority of people do not get health care
as intended.

We are well aware that the government in carrying out its mission to provide
quality health care standards, easy and affordable to the people certainly
can not move themselves, it takes the participation of various parties
involving the role of professionals in the field of health for the realization of
the mission.

II. INTRODUCTION.
Independent Living Well Foundation is a social organization formed by
college "Baitul-Ibadah" in which one of the scope of activities engaged in
social religion. Domiciled in RT.004 RW.03, Pondok Petir Village, District
Bojongsari, Depok Municipality.

Based on the existing conditions in the area of Village cottage Lightning at

this time, to meet the health care needs of the community in a cheap and
affordable is still perceived to be added both the means and infrastructure,
so that people no longer constrained in obtaining health services in terms of
both mileage and costs.

As the intent of this proposal, an attempt to establish a health care unit for
people, especially in the area of Surau Pondok Petir Village This stems from
the idea of a drg. Hamka Hasan who intend either donate their knowledge in

the field of medicine for the benefit of the society, especially the area Surau
Pondok Petir.

In accordance with the vision and mission of the YAHIRA Foundation then
together with drg. Hasan Hamka we are planning to establish a public health
care units are cheap and affordable, in the form of partnerships.

Furthermore, the public health services would be named "Baitul-RAHIM"
which consists of Dentistry Clinic and Polyclinic General.

III. VISION, MISSION, PURPOSE AND OBJECTIVES.
