JOINT AFFIDAVIT
WE, ROSITA DOMINDING and ANGELITA FADERAGAO, both of
legal ages and residents of Brgy. Calicanto, Batangas City, hereby under oath depose
and say:
That we personally know one Diocyn G. Capili, he being one
of the legitimate heirs of Spouses Dionisio Capili and Cynthia GeronCapili;
That aforesaid Diocyn G. Capili was born on September 15,
1987;
That it appears from his Philhealth Identification Card that his
name was written as Dinain G. Capili instead of Diocyn G. Capili;
That Dinain G. Capili and Diocyn G. Capili refer to one and
the same person;
That we are competent to state the foregoing circumstances
because we are closely intimated with his family, having lived in the
same place since his birth;
That we are executing this affidavit to attest the truth of all the
foregoing statements and for whatever legal purpose it may serve.
