Dear Maam/Sir,
Good day! I am Desiree Ann I. Fernandez with student number 20110590,
graduate of Bachelor of Science in Criminology last December 20, 2014 of ICCT
Colleges. Please accept my deepest apology for violating the rules of our school in
tampering and removing stickers on my transcript of records (T.O.R) to take the board
exam directly without passing the school pre-board exam. It was not my intention to
make bad image to the name of our school me and my colleague decide to go to a stall
in Recto in front of PRC to remove and tampered our TOR for eagerness to take a
board exam immediately. Rest assured that ICCT Colleges-Office of the Registrar had
no idea on this matter and I am the one who initiate to do this. With this regard, May I
also request for my pending official documents (Diploma and Transcript of Records) to
release.
