The Graduate Education Scholarships for Faculty and Staff (CMO 3, s. 2016) has two goals: to mitigate
impact on labor and to upgrade qualifications of faculty. The Commission through the K to 12 Transition
Program is offering full scholarships for masters and doctoral degrees for faculty and staff during the
transition period. Higher education institutions (HEIs) are invited to nominate their qualified faculty and
staff to earn graduate degrees.
This page contains forms for the scholarship nomination, guidelines via pertinent CMOs, and
presentations from workshops in relation to the graduate scholarships.

Curriculum Vitae (using template)

One-page thesis/dissertation proposal (faculty with ongoing


Masters/Doctoral studies)

Re-entry plan and return service contract

DBP form and 11 photo

Proof of citizenship (NSO authenticated birth certificate,


information page of valid passport, voters ID) (send only one;
photocopy)

Medical certificate issued by a government physician within the

last 6 months and medical exam results (original)

Workshop Presentation
The K to 12 Transition Project Management Unit traveled around the Philippines to conduct nomination
workshops that will inform higher education institutions (HEIs) on effectively nominating their faculty
and staff for graduate scholarships this June SY 2016-2017.

Delivering HEIs Assembly


The delivering HEIs (DHEIs) assembly was held last June 1, 2016 at the NCPAG Assembly Hall in UP
Diliman. Updates on the scholarships, DHEI responsibilities, the grants management office, and next
steps were discussed during the assembly.

Scholars On-boarding and SHEI Orientation


All approved nominees and representatives from their sending higher education institutions (SHEIs) are
required to attend the Scholars On-boarding and SHEI Orientation to be held in their respective regions
in June and July 2016.
