AFFIDAVIT OF UNDERTAKING

I, Clifford Glenn D. Malaay of legal age, Filipino, with office address at TAPI
Bldg., DOST Compound, General Santos Avenue, Taguig City after having been duly
sworn to in accordance with law, hereby depose and state:
1. That I am a employed as Science Research Specialist II at the Legal Unit of
the Technology Application and Promotion Institute (TAPI);
2. That TAPI filed a case against delinquent inventors who have availed of the
Inventors Guarantee Fund under RA 7459;
3. That TAPI need to borrow the original document of the Appraisal Report of
Yrezabal and Company Incorporated dated June 17, 2004 for pre marking
in court which is under the custody of the Development Bank of the
Philippines;
4. That I hereby undertake to comply with the duty to preserve, maintain and
return the aforementioned document as soon the purpose intended is
served or right after the marking in court;
5. That this affidavit is executed in order to attest to the truthfulness of the
foregoing narration of facts and for whatever legal intents and purposes it
may serve.
