Philippine Passport issued on _____, issued by the Department of
Foreign Affairs;
2. That I neglected to put my picture on my said Individual Taxpayers
Identification Card due to inadvertence and it was only recently when
I learned that I need to make it happen;
3. That I used the name ___________ in all my official records;
4. That the name _____________ indicated in my Philippine Passport
and the same name indicated in my BIR TIN Identification Card abovementioned, refer to one and the same person;
5. That I am executing this affidavit to attest to the truth of the foregoing
facts and for whatever legal purpose/s it may serve.
