AFFIDAVIT OF TWO DISINTERESTED PERSONS
We, CELSO S. QUILING currently employed as Project
Development Officer (PDO) III
and PERLITA I. QUILING currently
employed as Administrative Assistant III of Palawan Council for
Sustainable Development Staff (PCSDS), both of legal age, Filipinos,
and residents of the City of Puerto Princesa, after having been sworn
in accordance with law, hereby depose and state that:
1.
We know personally the Spouses FERNANDO RIVERA FLOR
(now deceased) and JEOMIHA DALINDIG EBOG who was born on 23 July
1956 in Tagpait, Aborlan, Palawan. We are their friends and as such we
had the privilege to know their personal circumstances and that of
their family.
2.
We know the fact that in her birth certificate her full name
was recorded as JEOMIHA DALINDIG EBOG which is the correct one.
3.
We know also for a fact that in the marriage contract of
JEOMIHA DALINDIG EBOG, her last name was erroneously entered as
DALINGDI
G instead of DALINDIG.
4. Similarly, her name was inadvertently entered in her SSS
Personal Record (SSS form E-1) as JOEMEHA E. FLOR instead of
JEOMIHA E. FLOR which is the correct one.
5.
That JEOMIHA DALINDIG EBOG, JEOMIHA DALINGDIG EBOG
and JOEMEHA E. FLOR refer as one and the same person.
6.
We are not related to JEOMIHA DALINDIG EBOG either by
affinity or consanguinity.
7.
We are executing this affidavit to attest to the truth of the
foregoing, to prove that the names JEOMIHA DALINDIG EBOG,
JEOMIHA DALINGDIG EBOG and JOEMEHA E. FLOR refer to one
and the same person, and for whatever legal purpose it may serve.
