Situation:
You are the public health nurse at the rural health unit. In the
garantisadong pambata (outreach health services) visit at an ambulatory
clinic in Bo. 2, you met Jenny Reyes, she is 26 years old, married and she is 7
months pregnant with her first child.
You found out that Jenny never had a prenatal check-up. You also noted
that she was underweight of only 48 kg and a height of 155 cm. when you
asked her where she plans to deliver her baby, she replied that she would
probably have a home delivery under the care of local hilot because
professional attendance would be too expensive for them. She explained to
you that she came to ambulatory clinic upon the prodding of her husband
who heard about the health workers visit to the barangay. To assess the
Reyes home situation and to teach jenny health practices related to her
pregnancy, you asked Jenny if you could make a home visit. Seemingly
pleased with your attention, Jenny gave her permission for you to make a
home visit.
When you made the home visit you noted that Jenny lived with her 33 year
old husband Mario, who was at work at the time of visit. He was the sole
breadwinner of his family- a construction worker earning a minimum wage.
Currently he has a cough. Jenny described her husband as hardworking. They
lived in a rented shack of mixed materials with a bedroom, a bathroom and a
toilet and a small multi-purpose room (living and dining room and kitchen).
Jennys activities consisted mainly of household chores. Sometimes Jenny
would spend time at homes friends and relatives residing in the
neighbourhood.
In the course of interview, you found out that Jenny had an inadequate
knowledge about community health services, prenatal nutrition, preparation
for childbirth and infant care. She said that sometimes she and her friends
and relatives talked about such matters, but the information given was
confusing and conflicting.
Aside from BP of 160/110, edema in the hands and underweight, other
findings during physical exam were normal. When asked about her diet,
Jenny told you that she limited her food intake because she did not want to
have a caesarean section, which may be needed if the baby grew too big.
