AFFIDAVIT
I, GRACE AMPOYAS CABAA, of legal age, married to Gerry G. Cabaa and
a resident of Galinato Village, Isulan, Sultan Kudarat, after having duly sworn to in
accordance with law, do hereby depose and state:
1.
That I am the registered owner of that certain parcel of land identified
as Lot 531-A-11, Psd-12-033685, sistuated in the Barrio of Kalawag, Isulan, Sultan
Kudarat, containing an area of One Hundred Eighty Five (185) Square Meters, more or
less, and covered with Transfer Certificate of Title No. T-46258;
2.
That I have sold the above-described parcel of land in favor of HAZEL
MELVINA SY SIO as evidenced by a Deed of Absolute Sale duly notarized by Notary
Public Herculano T. Tagaloguin and entered in his notarial registry as Doc. No. 445, Page
No. 89, Book No. IV, Series of 2002;
3.
That I have already given written notice to the owner of the adjoining
lots of the said parcel of land and to all possible redemptioners with regard to the sale of
the same;
4.
That said notice apprised the said owner of the adjoining lands with
regard to their rights of the redemption in case they wish to redeem the same from the
herein Vendee;
5.
That I am executing this affidavit pursuant to the provisions of Article
1623 of the New Civil Code of the Philippines as pre-requisite to the registration of the
aforementioned Deed of Absolute Sale.

JOINT AFFIDAVIT
We, PONCIANO T. TURCO and HENRY P. VIDAL, both of legal age, married
and residents 844 Bolton Ice Plant, Davao City and Brgy. Tina, Tacurong City,
respectively, after having duly sworn to in accordance with law, do hereby depose and
state:
1. That we personally know the spouses ROSARIO T. TAMAYO and
BONIFACIO D. TAMAYO of Bolton Ice Plant, Davao City;
2. That the said spouses were blessed with children and one of them is
GABRIELLE T. TAMAYO whom we also know personally to have been born on August
22, 1996 in Davao City by the assistance of a trained hilot;
3. That said birth of Gabrielle T. Tamayo was not registered by her parents with
the Office of the Local Civil Registrar of Davao City due to neglect and inadvertence;
4. That to date, the said child was put under the custody of her adoptive parent;
5. That we are executing this affidavit to attest to the truthfulness of the abovestated facts and circumstances for the purpose of enrolling the said child to a recognized
Elementary School in Davao City.

That on November 19, 2002 at about 9:45 P.M., I
personally saw P01 Gilbert Aronales having a fist fight with Johnny Gumbao, Sammy
Aquino and Ricky Alpas near the satellite market in Silway 8, Polomolok, South
Cotabato;
2.
That I have seen how P01 Aronales was
outnumbered/over-powered by his opponents making him to flee to evade serious bodily
harm on his person;
3.
Aquino;

That he was chased by Johnny Gumbao and Sammy

4.
That I have seen Johnny Gumbao going home and
returned with a 38 caliber revolver still looking for P01 Aronales;
5.
That he uttered some threatening remarks which is a
threat to the life of P01 Aronales;
6.
That I am executing this affidavit to attest to the truthfulness of the abovestated facts and circumstances and for whatsoever legal intent it may serve best.
