The Retirement and Later Years stage

The main tasks are coping with physiologic decline in the self as manifested by the illness of Mr. Ricardo and his
inability to work, adjusting to children taking a more central role in day to day activities like household work, valuing
the wisdom and experience of the elderly as their children still rely on the advise of their parents with regards to
decision making, dealing with the loss of a peer or siblings, and reviewing, reminiscing and preparing for ones own
eventual demise. The key emotional trait is accepting the shift in generational roles. Obviously, the chances of
having a chronic physical illness or a psychological one increases with age like in the case of Mr. Ricardo who has an
endocrine disease. Degenerative conditions and debility like Mr. RicardosDiabetic Retinopathy are also expected to
occur.
FAMILY ILLNESS TRAJECTORY
There are four stages that the family undergoes in coping with illness (Leopando, 1999 and Lavina 2002). The
timeline which the family surpasses each stage depends on the factors. At each stage the doctor forearmed with
knowledge of what the family members are gointhrough and what they need to know can be staunched
therapeutically and a source of extra-familial support. (Textbook of Family Medicine)
Stages Trajectory of Illness
Stage 1
Onset of symptoms and illness
Stage 2
Impact phase reaction to the diagnosis
Stage 3
Major therapeutic efforts
Stage 4
Recovery phase adjustment to outcome
The patient is categorized as Stage 4 because the patient is currently adjusting with the outcome of the disease. The
patient manifests signs and symptoms and the complications of Diabetes Mellitus. Patient is on maintenance
medications but activities of daily living are still affected.

Role of the Physician

Physicians play a diverse role in dealing with a family struck with illness and it is therefore important that they play
an integral part in aiding the family to reach wellness. Some of the task that the physicians needs to perform are as
follows:
- Monitor patients condition
- Advise the patient to seek medical help if necessary
- The physician should advise the family to monitor his vices ( Drinking Alcohol)
- The physician should advise the patient to have a proper diet
- Life style modification regimens such as exercises to improve his breathing
