How to Receive Your Rebate

Please COMPLETE and MAIL all required supporting documents and the following forms:

■ A Rebate Application

■ A Proof of Recycling Certificate for the old appliance model

■ A copy of the purchase receipt OR an original rebate receipt for the new appliance model

■ The original, bright yellow ENERGY GUIDE tag or label from the new appliance model

■ A
 copy of your most recent utility bill dated 60 days or less from the appliance purchase date, which must
show your residential street address (for proof of residency)

All applications must be postmarked within 30 days of purchase date of appliance. Post office box addresses do
not qualify as a valid residential address for this program.

Incomplete and incorrect applications cannot be processed and will be rejected.

The California Cash for Appliances Program reserves the right to inspect and confirm appliances and their
installation.
