BIPARIETALDIAMETERANDHEADCIRCUMFERENCE
TheBPDandOFDaremeasuredonatransverseaxialsectionofthefetalheadwhichincludesthe
falxcerebrianteriorlyandposteriorly,thecavumseptumpellucidumanteriorlyinthemidlineandthe
thalami.TheBPDismeasuredfromtheouteredgeofthenearerparietalbonetotheinneredgeof
themoredistantparietalbone.TheOFDismeasuredperpendiculartotheBPD.
FEMURANDHUMERUSLENGTH
Thelongbonesaremeasuredwiththeboneacrossthebeamaxis.Thestrongacousticshadow
behindthefemoralorhumeralshaftandthevisualisationofbothcartilaginousendsindicatesthatthe
imageplaneisonthelongestaxisandistheoptimalmeasurementplane.Thecalipersareplaced
alongthediaphysealshaftexcludingtheepiphysis.
ABDOMINALCIRCUMFERENCE
Thesemeasurementsaremoreappropriatelyusedintheassessmentoffetalgrowth,particularlyin
thesecondhalfofthepregnancy,thanintheassessmentofgestationalage.Itis,however,an
appropriatemeasurementinthesecondtrimestertodemonstratenormalfetalproportions.
Theabdominalcircumferenceismeasuredattheleveloftheliverandstomach,includingtheleft
portalveinattheumbilicalregion.

UltrasonicFetalMeasurementStandards
Ifweassumeafetusisgrowingnormally,biometricmeasurementsaredeterminedby
gestationalage,andwecanestimatethegestationalageandthustheduedate.Whenthe
dateofthelastmensesprecedingconceptionisknown,itisusuallyamoreaccurate
estimate(+/5days)ofEDC
Whenusingultrasoundtoestimategestationalage,rememberthatweareassumingthe
fetusisgrowingnormally.Conditionswhichalterfetalgrowthwillmaketheestimatesless
reliable.Althoughmanyembryonicandfetalstructurescanbemeasured,onlyafew
measurementsareeasyandrepeatableenoughforwidespreaduse.
Themostcommonare:
GestationalSac:
Thefirstelementtobemeasurableisthegestationsacoftheearlypregnancy.The
gestationalsacismeasuredinthreedimensions,andtheaverage,theMeanSacDiameter
(MSD)usedforestimatinggestationalage.Itisusefulbetween5and8menstrualweeks
withaccuracyof+/3days.Asaroughruleofthumb,theMSD+30=MenstrualAgein
days.
EmbryonicCrownRumpLength:
Thelengthoftheembryoonthelongestaxis(excludingtheyolksac)constitutesthecrown
rumplength.Thisisamongthebestdocumentedparameterstodatetheembryo,with
accuracyof+/35days.Asaroughruleofthumb,theCRL+6.5=MenstrualAgein
Weeks.
Biparietaldiameter(BPD):
Thetransversewidthoftheheadatit'swidest.Wemeasurefromthetheleadingedgeto
leadingedgeofthebones,becausethisleadinginterfaceismostdistinct.Sincetheheadis
oval,theerrorinducedbysmallerrorsinpositioningissmall,makingforarepeatable,robust
measure.
Headsizeisdeterminedlargelybybraingrowthwhichisrelativelyindependentofnutritional
(maternal/placentalinsufficiency)growthretardingprocesses,andheadgrowthisoften
relatively"spared"insuchgrowthretardation.
TheBPDbestusedafter12weeks.
Accuracyis+/1.1week1420weeks,
+/1.6weeks2026weeks,
+/2.4week2630weeks,
and+/34weeksafter30weeks.
FemurLength:
ThefemurlengthisarepeatablemeasurementwithaccuracysimilartotheBPD.Itis
effectedbyskeletaldysplasias,butsincethesearerare,itisareliablemeasurementwhich
confirmsmeasurementsofthehead.Itisbestmeasuredafter14weeks.
Itiscommontomakeatleast2andoften45measurementstoestimategestationalage.In
mostcases,BPD,HeadCircumference,Femurlength,andAbdominalcircumferenceare
used.
Sinceallofthesemeasurementsarestronglyrelatedtogestationalage,itisnotusually
importanthowtheyarecombined.Mostmodernultrasoundmachinesincludecomputerized
biometricanalysisprogramsusedtoeasilycalculateyourEstimatedDueDateorEDDor
EDC.

Asapracticalmatter,ultrasoundscanninghasproventobesopopularwithpatientsand
alsotheirobstetricians,thatalmosteveryonereceivingregularprenatalcareendsupwithat
leastonescananyway.Thetotalnumberofscanswillvarydependingonwhethera
previousscanhasdetectedcertainabnormalitiesthatrequirefollowupassessment.
Thereisnohardandfastruleforthenumberofscansyoushouldhaveduring
pregnancy.
Insomecountries,sonogramsareperformedjusttwiceduringpregnancy.Onceat1618
weekstoassessfetalabnormalities,andagainat3234weekstoassessageandwell
being.
Itisnowrecommendedthatallpregnantwomenhaveadatingscaninthefirsttrimester
ideallyat10to13weeksofpregnancytoconfirmyourdates.Thisisespeciallyimportantif
youaregoingtohaveanyscreeningtestsforDown'ssyndrome,asknowingtheexactdates
makessureyourresultisaccurate.Mosthospitalsofferascaninthesecondtrimesterat
about20weeks(theanomalyscan)tocheckthatyourbabyisdevelopingnormally.
