To apply for the Biodesign Innovation Fellowship, please:
1.

Fill out the online application forms

Upload supporting material

3.

Designate three people to submit recommendation letters

4.

Pay an application fee of $85

Application Deadline is August 31, 2015

The online application must be submitted by this date, along with all supporting materials and
recommendation letters.

Application Submission
The application starts with a series of forms that must be completed online. You need not
complete them in one sitting; you will be able to save your work and return once you complete
the initial registration. To accessa an in-progress application, go to the original URL and login.
The following information will be required:
Name, address, phone number, email address
Current organization/employer, gender, date of birth, ethnicity (optional). We are also

interested in how you heard about our program and if you have applied for the fellowship in
prior years (and when).

Your educational history, including institution name and location, major and degree. Please

provide this for your two most recent programs, with the most recent first.

To submit your application:

Complete the forms and review your information for accuracy. Edit where necessary.
Upload the required supporting material (see details below).
Provide information about three people who will provide letters of recommendation (name,

title, institution, phone, email, relationship and a personal note from you requesting the
reference). Ideally, one letter should be from your most recent program director or manager.

Provide a video essay or link to the video on YouTube or Vimeo (see details below).
Submit your application once you have verified that everything is correct.

Supporting Material
After completing the forms, you will need to upload the following materials:
Your resume or curriculum vitae (details below)
Copy of transcript from your two most recent degree programs
Copy of national/state boards or qualifying examinations youve taken in the past 5 years

(scores from the latest exam(s) you have taken).

Your written application essay (details below).

Your video application essay (details below) .

Resume or Curriculum Vitae

Please upload a copy of your resume or curriculum vitae. It should include the following
information:
Your educational history.
Any national/state board qualifying exams.
Health occupation licenses or registrations .
Awards and honors.
Memberships in organizations.
Publications.
Intellectual property (patents or pending patents).
Primary research and scientific/clinical interests.
Summary of teaching experience.
Chronological listing of work experience including the organizations activities, your job title

and responsibilities, and start and end dates.

Written Essay
We request a two-page written essay in order to learn more about the person behind the

application (minimum font size 11). We encourage you to share with us your passions, values
and vision for your career.
Why is medtech innovation an important part of your career pathway?

Video Essay
Create a 1-2 minute video. Note: Do not discuss any specific IP you have developed if it has

not been patented.

Describe how your professional and personal experiences are unique.

Describe one experience that demonstrates your creativity and/or inventiveness.
Upload the video to the application website, or put the video on YouTube or Vimeo and

provide the URL.

Application Fee

There is an $85 non-refundable application fee to apply for the Fellowship that will be due by
credit card or PayPal at the end of the application. This fee is waived for current Stanford
students, sta and faculty - if you are currently at Stanford, do not pay the fee, but notify
andread@stanford.edu and she will issue a waiver in the system for you. Note: Your application
will not be considered until the $85 fee is received or waived. We apologize that we are not able
to accept other forms of payment.
