Note: Please submit this request to the Office of Food Quality Assurance at least 3 days
before the Event/Activity (Cut Off Time is 5PM, M-F). Please make sure to submit
endorsed documents only and fill up this form completely and legibly. Any changes/
additional information in the Form should be done at least 2 days before the
Event/Activity. The University has the right to stop a food catering event and remove
the food outlet caterer from the list of recognized food outlets due to unsanitary
practices of the food outlet and non-compliance to University FSQA and AEMC Policies.
