Contract of Employment

KNOW ALL MEN BY THESE PRESENTS:

This Contract made and entered into this __________________in City of Manila,
by and between:
White Cross (Wellness & Therapeutic Services), a single proprietorship,
with business address at No. 314 B. Mabini St. Sampaloc , Manila City herein represented
by Jerson Bungalso, hereinafter referred to as the President.
and
_____________________, legal age, Filipino citizen and with residence address
at ______________ ____________________________________________, hereinafter referred to as
the EMPLOYEE.
WITNESSETH:
WHEREAS, the EMPLOYEE is desirous of rendering services with the
EMPLOYER, under the following terms and conditions:
TERMS AND CONDITIONS:
That, this Contract of Employment shall be for a period of Two (2) Months
beginning ____________ ____________________________ renewable upon mutual agreement
of both parties;

That, in the case the Employer decides to renew a Contract of Employment


with the Employee, the Employee shall be notified thirty (30) days before the expiration
of this Contract of Employment;

That in the event that the Employee decides to resign from his/her work, the
Employee shall give the Employer due notice equivalent to two (2) months prior tom the
date of resignation;

That, in the event of non-completion of this two (2) months contract, the
Employee shall pay the Employer penalty charges amounting to TWENTY THOUSAND
PESOS (P20,000.00), to cover all the expenses incurred by the Employer during the
Employees period of employment;
That the Employee shall post cash bond in the sum of TWO THOUSAND
PESOS (P2,000.00) upon signing of this Contract , such deposit or cash bond shall not
be refundable to the Employee upon the expiration of the Contract after deducting all
outstanding liability due to the Employer and utilities , if any;
That the Employer shall pay the Employee the amount of Basic Salary plus
8%

That, the Employer is not liable for any personal financial transactions
undertaken / made by the Employee with any of its co-members and client;
That, the Employer shall assume full responsibility for any and all damages /
losses that may occur to the properties of the Employer issued and/or assigned to the
Employee necessary in their duties;
That, the Employer and the Employee should have respect for each other as
well as for their co-members. The Employee should know that the Employer has at all
times the right to impose things whenever needed for the benefit of all;
That non-compliance or violation of any of the terms and conditions of
Contract of Employment shall be sufficient cause for the termination of the Contract of
the Employee. In such case, the Employer shall have the rights to forfeit all security cash
bond of the Employee. In case of legal actions becomes necessary, the Employee shall
additionally be held liable for cost of suit, attorneys fees and liquidated damages.
