CorcomCommunicationsprovidesanyservicestowhichyousubscribe.Anydenialorrestrictionofyourapproval
remainsvaliduntilyourservicesarediscontinuedoryouaffirmativelyrevokeorlimitsuchapprovalordenial.
Insomeinstances,CorcomCommunicationswillwanttoshareyourCPNIwithitsindependentcontractorsand
jointventurepartnersinordertoprovideyouwithinformationaboutCorcomCommunicationscommunications
relatedproductsandservicesorspecialpromotions.PriortosharingyourCPNIwithitsindependentcontractorsor
jointventurepartners,CorcomCommunicationswillobtainwrittenpermissionfromyoutodoso.

CUSTOMERAUTHENTICATION
FederalprivacyrulesrequireCorcomCommunicationstoauthenticatetheidentityofitscustomerpriorto
disclosingCPNI.CustomerscallingCorcomCommunicationscustomerservicecentercandiscusstheirservicesand
billingswithaCorcomCommunicationsrepresentativeoncethatrepresentativehasverifiedthecallersidentity.
TherearethreemethodsbywhichCorcomCommunicationswillconductcustomerauthentication:
1)ByhavingtheCustomerprovideapreestablishedpasswordand/orPINtoacustomerservicerepresentative.
2)BycallingtheCustomerbackatthetelephonenumberassociatedwiththeservicespurchasedifapassword/or
PINhasbeenforgotten.
3)BymailinganyrequesteddocumentsonlytotheCustomersaddressofrecord.Anyotherelectronicmeansof
providingthisinformation(ie:email,text,facsimile,etc.)isnotpermitted.Thisisforrecordsrequestonlyandnot
asanalternativeforobtaininglostorforgottenpassword/orPINinformation
Passwordsand/orPINsmaynotbeanyportionoftheCustomerssocialsecuritynumber,mothersmaidenname,
amountortelephonenumberassociatedwiththeCustomersaccountoranypetname.IntheeventtheCustomer
failstoremembertheirpasswordand/orPIN,CorcomCommunicationswillasktheCustomeraseriesofquestions
knownonlytotheCustomerandCorcomCommunicationsinordertoauthenticatetheCustomer.Insuchan
instance,theCustomerwillthenestablishanewpassword/PINassociatedwiththeiraccount.

NOTIFICATIONSOFCERTAINACCOUNTCHANGES
CorcomCommunicationswillbenotifyingcustomersofcertainaccountchanges.Forexample,wheneveranonline
accountiscreatedorchanged,orapasswordorotherformofauthentication(suchasasecretquestionand
answer)iscreatedorchanged,CorcomCommunicationswillnotifytheaccountholderbyeithertheemailaddress
thattheyprovidedorbymailingthenotificationtotheiraddressofrecord.Additionally,afteranaccounthasbeen
established,whenacustomersaddress(whetherpostaloremail)changesorisaddedtoanaccount,Corcom
Communicationswillalsosendanotification.

DISCLOSUREOFCPNI
CorcomCommunicationsmaydiscloseCPNIinthefollowingcircumstances:

WhentheCustomerhasapprovedtheuseoftheirCPNIforCorcomCommunicationsorCorcom
Communicationsjointventurepartnersandindependentcontractors(asthecasemaybe)salesor
marketingpurposes.
Whendisclosureisrequiredbylaworcourtorder.
ToprotecttherightsandpropertyofCorcomCommunicationsortoprotectCustomersandothercarriers
fromfraudulent,abusive,orunlawfuluseofservices.
WhenacarrierrequeststoknowwhetheraCustomerhasapreferredinterexchangecarrier(PIC)freeze
ontheiraccount.

ToprovidetheservicestotheCustomer,includingassistingtheCustomerwithtroublesassociatedwith
theirservices.

PROTECTINGCPNI
CorcomCommunicationsusesnumerousmethodstoprotectyourCPNI.Thisincludessoftwareenhancementsthat
identifywhetheraCustomerhasapproveduseofitsCPNI.Further,allCorcomCommunicationsemployeesare
trainedonthehowCPNIistobeprotectedandwhenitmayormaynotbedisclosed.Allmarketingcampaignsare
reviewedbyaCorcomCommunicationssupervisorycommitteetoensurethatallsuchcampaignscomplywith
applicableCPNIrules.
CorcomCommunicationsmaintainsrecordsofitsownanditsjointventurepartnersand/orindependent
contractors(ifapplicable)salesandmarketingcampaignsthatutilizeCustomerCPNI.Includedinthis,isa
descriptionofthespecificCPNIthatwasusedinsuchsalesormarketingcampaigns.CorcomCommunicationsalso
keepsrecordsofallinstancesinwhichCPNIisdisclosedtothirdpartiesorwherethirdpartieswereallowedaccess
toCustomerCPNI.
CorcomCommunicationswillnotreleaseCPNIduringcustomerinitiatedtelephonecontactwithoutfirst
authenticatingtheCustomersidentityinthemannersetforthherein.ViolationofthisCPNIpolicybyanyCorcom
CommunicationsemployeewillresultindisciplinaryactionagainstthatemployeeassetforthinCorcom
CommunicationsEmployeeManual.
BREACHOFCPNIPRIVACY
IntheeventCorcomCommunicationsexperiencesaprivacybreachandCPNIisdisclosedtounauthorizedpersons,
federalrulesrequireCorcomCommunicationstoreportsuchbreachestolawenforcement.Specifically,Corcom
Communicationswillnotifylawenforcementnolaterthanseven(7)businessdaysafterareasonable
determinationthatsuchbreachhasoccurredbysendingelectronicnotificationthroughacentralreportingfacility
totheUnitedStatesSecretServiceandtheFBI.Alinktothereportingfacilitycanbefoundat:
https://www.cpnireporting.gov.CorcomCommunicationscannotinformitsCustomersoftheCPNIbreachuntilat
leastseven(7)daysafternotificationhasbeensenttolawenforcement,unlessthelawenforcementagenttells

thecarriertopostponedisclosurependinginvestigation.Additionally,CorcomCommunicationsisrequiredto
maintainrecordsofanydiscoveredbreaches,thedatethatCorcomCommunicationsdiscoveredthebreach,the
datecarriersnotifiedlawenforcementandcopiesofthenotificationstolawenforcement,adetaileddescriptionof
theCPNIbreach,includingthecircumstancesofthebreach,andlawenforcementsresponse(ifany)tothe
reportedbreach.CorcomCommunicationswillretaintheserecordsforaperiodofnolessthantwo(2)years.

NOTIFICATIONOFCHANGESTOTHISPOLICY
IfwechangethisCPNIPolicy,wewillpostthosechangesonhttp://www.LUSFiber.comorinotherplacesthatwe
deemappropriate,sothatyoucanbeawareofwhatinformationwecollect,howweuseit,andunderwhat
circumstances,ifany,wediscloseit.Ifyoudecidetocontinuereceivingyourservicesafterwemakeanychanges
tothistheCPNIPolicy,youshallbedeemedtohavegivenyourconsenttothechangesintherevisedpolicy.
