Employment:
2007 to Current Choices Network (previously Magellan Health) Position: Case Manager
Member of the clinical team, complete assessment and service plans and implement services to consumers in
accordance with their individualized service plan (ISP). Communicate and complete documents reporting consumers’
progress toward their recovery. Work collaboratively with the clinical team to engage, educate, communicate, and
coordinate care with consumer, their family, behavioral health, medical and dental providers, community resources and
others in ensuring that all services prescribed in the individualized service plan are implemented.

8-2005 to 2007 Southwest Key Program Position: Case Manager


Work as a case manager for a direct support services program. Worked with adolescents teaching various skills such as
life skills, social skills, anger management, coping techniques, substance abuse prevention and intervention, positive
self esteem, community outlets, daily structure, mentoring, parenting skills, appropriate boundaries, and appropriate
relationships. Develop effective service plans as well as execute the plan. Facilitate Girls group Identification and
Expression for females 13-17 teaching effective communication skills, personal expression, skill to identify needs,
problem solving skills and appropriate feminine behavior and manners. Provide case notes and maintain client file.
Adhere to policy and procedure of program. Interact with Value options representatives, Juvenile Probation
department, CPS, DES, and various agencies to ensure the progress of service plan.

9-2003 to 8-2005 B J Group Homes Position: Program Manager


Work as an administrator for a therapeutic group home. Provide clients with structured activities as well as life skills,
