JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
We, REYNARIO M. REDONA and THELMA C. OCANA, both of legal ages, Filipinos, married, and
residents of Brgy. Cabuynan, Tanauan, Leyte, after having been duly sworn according to law, hereby depose and
say; THAT:
1. Being the close acquaintances and neighbors, we personally know the one named JEANNIE
BALICONG CARREON of Brgy. Cabuynan, Tanauan, Leyte who is our close friend and
neighbor;
2. That we have the personal knowledge, and fully aware of the fact that she died on November 8, 2013 at
Brgy. Cabuynan, Tanauan, Leyte at the height of the storm surge brought about by the catastrophic
Super Typhoon Yolanda;
3. That her fact of death was registered with the office of the Local Civil Registrar with the complete name
JEANNIE BALICONG CARREON, the name entered as she died as an unmarried or solo parent;
4. We have the full knowledge of the fact that she, during her lifetime, was a regular member/depositor of
PERPETUAL HELP CREDIT COOPERATIVE, Tacloban City, of which name she registered was
JEANNIE C. REDONA of Brgy. Cabuynan, Tanauan, Leyte, purportedly using the surname
REDONA, the surname of her live-in husband whom she had been cohabitating as married couple
without the benefit of marriage;
5. That the above-cited discrepancies we believe to be a clear case of misapprehension of facts brought
about by an honest mistake;
6. Thusly, we hereby affirm and confirm the fact that the names:
a) JEANNIE BALICONG CARREON of Brgy. Cabuynan, Tanauan, Leyte, as the name
appearing in her Certificate of Death; and the name,
b) JEANNIE C. REDONA of Brgy. Cabuynan, Tanauan, Leyte, as the regular
member/depositor of PERPETUAL HELP CREDIT COOPERATIVE, Tacloban City;
REFER TO ONE AND THE SAME PERSON, as herein-named identified subject person, that of
JEANNIE BALICONG CARREON of Brgy. Cabuynan, Tanauan, Leyte, as the subject member/depositor
of PERPETUAL HELP CREDIT COOPERATIVE, Tacloban City;
7. That We have freely and voluntarily executed the foregoing affidavit to the attest to the veracity of
foregoing narration of facts without any mental reservation or purpose of evasion nor to defraud
creditors or any third person, but to ATTEST, AFFIRM and CONFIRM the foregoing statement of facts,
and for such any legal and general purposes it may serve.
