Problem definition
It is the companys responsibility to provide a safe workplace environment to all employees regardless their organisational level, and
its responsibility to early detect potential violence or aggression.
and so workplace aggression or violence is failure of the company
to detect it earlier and acts accordingly. accordingly the case of
death and injury due to the entry of the x-employee with his gun is
the company responsibility.

Alternatives Evaluation
Psychological testing and behavioural interviewing to identify behaviours and experiences that might suggest further inquiry prior to
hiring an employee. Information on past criminal activity and financial and employment history may be investigated before hiring.
Companies should have a workplace violence policy that clearly
describes the commitment to a safe workplace, employees roles
and responsibilities, Definitions of unacceptable behaviour are described, along with possible ad- ministrative actions.
although the company is able to install metal detector that might
helped in detection of the shotgun, yet violence can be acted outside the company or by using another non metal object. and so a
security risk audit may include an employee survey about security
concerns and examination of current protective practices.
Establishment of internal threat management team, consisting of
representatives from human resources, legal and security. their role
is to assist the potential threats that may surface by periodic surveys.
Recommendations
It is important to conduct a lesson learned to improve the security posture of the organization. A lessons learned meeting should
include all personnel involved including management and provide
closure to the incident by reviewing what occurred, what was
done, what did and did not work well, and a plan of action to prevent a similar attack from occurring.
