Mr. Bush:

Its my understanding that the Criminal Court will not release any information to me about the Victim, i.e.
PACE. We do not generally pull criminal records and youre correct that a transcript of the hearing may be
useful, if it is still available. Before we take that route however, Ive attached an authorization for me to
contact your Public Defender, Tony Flemmer. I am hopeful he can provide the information we need to
proceed.

Ive also attached our Health Legal Services opening paperwork.

The Legal Representation Agreement explains that we will continue to investigate the Peaceable Contact
Order with PACE and, if necessary, contact PACE regarding the Order. As I explained before, we are not
able to file a personal injury or other tort claim on your behalf against the PACE Clinic. We are also not able
to represent you in an appeal if the County denies your Medical Managed Care Plan disenrollment.

Please review the Legal Representation Agreement carefully and contact me with any questions or concerns.
Please note that HLS cannot make any guarantee about the success of any action covered by the agreement, but
promise to put forth our best efforts to advance your case. Everything we discuss is confidential and covered
by the attorney client privilege. I ask that you keep me informed of any developments and be honest and
forthright about your case. If youre unsatisfied with our services, we do have a grievance procedure which is
also attached. You have the right to terminate this agreement at any time. HLS reserves the right to terminate
assistance if you fail to cooperate, a conflict arises, or another breach occurs. Lastly, we keep your file for 7
years and then destroy it.

Also attached is a Client Complaint Procedure explaining our process, the complaint form, an ARIES consent
for services and information explaining the ARIES system, and a Ryan White Consent for Services Form.
Please print, sign, and date, in the areas indicated.

If you have a Ryan White Card, please also scan and send a picture of both sides. If you do not receive Ryan
White Services, I need 3 pieces of information from you: 1) proof of diagnosis, 2) proof of income, and 3)
proof of residency.

Lastly, Ive attached HIPPA releases for both VMC and Stanford, should we need them.

Once I receive these documents back from you, I will promptly reach out to Mr. Flemmer.

notinanywayintrusiveordangerousorunreasonabletorequest.
Treatmentrightnowistheonlythingthatmattersit'swarranted,andfailuretoprovideitatallisactionable.

For the Aries/RW paperwork if you do not have a printout of your diagnoses and/or medical conditions, a

income, we just need a copy of your monthly benefit statement or your most recent recertification paperwork.
Yes, your California ID is sufficient to prove residency (assuming it lists your address in Santa Clara County,
if not, 2 copies of mail sent to the apartment youre staying will do).

I apologize for the necessity of all of this paperworkI know it can be difficult to gather up.

Once I receive the release and Legal Representation Agreement back from you, I will continue to investigate
the situation with PACE. We currently have a law clerk working at our office with experience at the PDs
office is it okay with you if he assists on your case? His name is Alvin Yu.

As far as treatment, can you please tell me a bit more about where/when/how you have been refused
treatment? You mentioned you were headed to Stanfords Positive Care Clinic earlier this week were you
able to receive treatment there? Is the VMC pharmacy refusing to fill some of your prescriptions?

I ask that you give us some more time to investigate the situation with PACE before pursuing injunctive relief.
