Please submit the following documents on 1st day of the workshop, in case you have not submitted.

1. Fee for the course Rs. 15,000/- (INR) or the remaining fee before 30th June 2008.
2. Application form.
3. Bio-data & two passport size colour photographs.
4. Proof of your Identity (Submit any one of the following documents)
Photocopy of IAP regd. Certificate/ from your Hospital.
Photocopy of your I-card from your Institution.
Photocopy of your Degree / Diploma.
Note: - If you have not submitted the complete application form (Please download it from
www.capri4physio.com) & above mentioned documents (it is a must). (Please ignore, if already sent).
Please note that other items like receipt of your payment (after realization of full payment), admit card
will be given at the venue at the registration desk before the course commences. Please inform us well
in advance, You are requested to report at the above venue at aprox. 8.30 AM on 1st day along with this
letter.
You must bring one small towel & come in loose dressing preferably in T-shirt (female participantswith deep neck from back & sleeve less) & half pants or parallels, with long hairs pulled back, finger
nails must be cut & trimmed. You must not wear any ring, wrist watch & ornaments. It is very strongly
recommended that during & after the workshop you must read Brian Mulligans 5th edn. (Latest edn.)
Book (manual) on Mulligan Concept to get maximum benefit out of this course. You are also strongly
advised to have your own self SNAG straps for Cervical / Lumbar spine to practice during & after this
