All staff nurses must attend the
endorsement
Stressed the importance of
endorsement to outgoing and ingoing
shifts. Active participation of every
member in the endorsement must be
observed.

Suggested that 15 minutes prior to

endorsement, ingoing shift must attend
to receive endorsement.
C. Documentation
Proper documentation stressed out
Informed consent
Informing IM concerns:
1. Informing Medicine preoperatively
is a factor for delay of wheeling
clients to Operating Room
2. Suggested that IM should be
informed after endorsement even
if the patient is not yet on call to
OR.
Everything that was done to the patient
and if there are eventualities regarding
patient status must reflect in the chart.
Writing legibly in the chart stressed out
Modifications regarding proper
medication administration and
preparation
D. Logbooks Monitoring
Adherence in filling up the logbooks
properly
Monitoring logbook must be observed
to meet the demands of quality control
Other Concerns:
Verification and referral of doctors order
that are unclear
See to it that laboratory request is the
same with the doctors order.
