Candidates should be aware and understand that the disclosure of any examination materials including
the nature or content of examination items, before, during, or after the examination is a violation of law.
A violation of this type can result in criminal prosecution or civil liability and/or disciplinary actions by
the licensing agency including the denial of licensure.

ADMISSIONS REQUIREMENTS

Authorization to Test (ATT)


You must present this ATT at the test center on the day of your appointment to be admitted to the
examination. You will not be admitted to the examination without this ATT and will be required
to re-register and re-pay to take the examination You must test within the validity dates printed on
this ATT. These validity dates cannot be extended for any reason.

Your Name
The printed first and last names on your identification must match exactly the printed first and
last names on this ATT. If your name on this ATT is different from the name on your identification, you
must bring legal name change documentation with you to the test center on the day of your test. The only
acceptable forms of legal documentation are: marriage license, divorce decree and/or court action legal
name change documents. All documents must be in English. All documents must be the original
document. Candidates wishing to make a legal name change must contact their board of nursing, and
then contact Pearson VUE to request a corrected ATT.

Identification Requirements
When you arrive at the test center, you will be required to present one form of acceptable identification
from the list below, and this ATT. You will not be admitted to the examination without the proper
identification and this ATT; if either of these requirements is not met, you will be turned away;
if you arrive without these materials, you will be required to re-register and re-pay to take the examination.

Acceptable Identification
Your identification must be printed in English language letters, must be valid and unexpired (clearly
stated), and have a recent photograph and signature signed in English. If you are testing at a Pearson
Professional test center located in any other country other than the United States (including its'
territories), you can only present your passport as your identification. No other identification will be
accepted at test centers outside of the United States. The only acceptable forms of identification at
United States test centers (including U.S. territories) are drivers license, state/province ID, passport,
United States military ID (with visible signature, not embedded), or national identity card. No other
identification, including learner permits, is acceptable for admittance to the NCLEX examination.

Please note - Identification from embargoed countries will not be accepted at the test centers. For further

TO SCHEDULE YOUR APPOINTMENT

You must schedule your examination within the validity dates listed above. These validity dates cannot be
extended, regardless of the reason. See your NCLEX® Examination Candidate Bulletin for detailed
instructions on how to schedule your appointment at a Pearson Professional Center. The NCLEX
Candidate Bulletin can be accessed at www.pearsonvue.com/nclex

Once you have scheduled your appointment, write the date and time of your appointment and the address
of the test center in the space provided. A confirmation of your appointment day and time, as well as
directions to the test center, will be sent to you.

Plan to arrive at the testing center 30 minutes before your scheduled appointment to complete admission
procedures before your testing begins.

If you fail to keep your appointment or do not re-schedule or unschedule your appointment at least 24
hours prior to your scheduled appointment, you will forfeit your examination fee and your Authorization to
Test (ATT) will be invalidated. You will be required to re-register and pay the examination fee again. There
are no exceptions for any reason. If you do not test within the validity dates as indicated on your ATT, you
will lose your NCLEX Examination fee. THESE VALIDITY DATES CANNOT BE EXTENDED,
REGARDLESS OF THE REASON. For further information see the NCLEX Examination Candidate Bulletin
or the NCLEX Candidate Web site.

If you need to change your appointment for any reason, you must do so at least 24 hours prior to your
scheduled appointment through one of the methods described above. Follow the schedule below if you
need to change your appointment.

For a test scheduled on Saturday, Sunday or Monday, call on or before the time of your appointment on
Friday.
For a test scheduled on Tuesday, call on or before the time of your appointment on Monday.
For a test scheduled on Wednesday, call on or before the time of your appointment on Tuesday.
For a test scheduled on Thursday, call on or before the time of your appointment on Wednesday.
For a test scheduled on Friday, call on or before the time of your appointment on Thursday.

PLEASE NOTE: Leaving a message on an answering machine does not constitute giving 24 hours
advance notice for rescheduling or un-scheduling your testing appointment. You must either reschedule or
un-schedule your appointment on the NCLEX Candidate website or speak with an individual on the NCLEX
Candidate Services line to reschedule or un-schedule your appointment.

YOU WILL NOT BE ADMITTED TO TAKE YOUR EXAMINATION WITHOUT YOUR AUTHORIZATION
TO TEST.
