HOME CARE STAFF NURSE
REPORTS TO:
Program Manager
Nurse Administrator
JOB DESCRIPTION:
The following duties are normal for this position. These are not to be construed as exclusive or allinclusive. Other duties may be required and assigned.
As a Home Care Registered Nurse, you will visit multiple patients homes each day and provide
them with a full range of personalized care, which may include such items as instruction on chronic
disease management and medication, pain management, wound care, IV infusion, and postoperative instruction. Part of your job is to work with patients and provide the tools they need to
ensure their own recovery. Seeing the end results of their recovery is just one of the many rewards
of your position.
Your specific duties as a Home Care Registered Nurse will include:
Providing case management as described in state regulations for all cases involving nursing
and therapy services
Making the initial evaluation visit as applicable and re-evaluating each patients nursing
needs on a regular basis
Providing nursing services, treatments and diagnostic procedures requiring specialized skills
and assuming full responsibility for each of your patients
Developing and implementing the treatment plan for each patient under the direction of
their physician
Observing and reporting symptoms, reaction to treatments, drugs, and changes in the
patients physical or emotional condition
Initiating preventative and rehabilitative nursing procedures as appropriate for the patients
care and safety. Effecting changes in environment to ensure patient safety and security.
Maintaining clinical and progress notes for each patient receiving care and providing
progress reports to their physician
Coordinating services for patients as needed to other agencies and health care providers as
appropriate.
Counseling the patient and their family in meeting nursing and related needs
Providing supervision of home health aide services and preparing written instructions for
care provided by home health aides
Assisting the Nurse Administrator and Home Care Program manager in decision making
and policy formation as it pertains to the Home Care Program.
Conserving agency resources and adequately maintaining agency property, supplies, and
equipment.
Adhering to all agency polices, including but not limited to, infection control and safety,
education, reporting and practice implementation.
CPR certified
Excellent communication skills, flexible and able to interact positively with the rest of the
staff.
Ability to see and hear adequately to respond to auditory and visual requests
Ability to travel as required. Current valid drivers license and automobile insurance.
This position has potential for cross training to other programs within the agency. Cross
training to perform those job functions is not included in the home care job description.
This portion of cross training will be utilized on an as needed basis.

While performing the duties of this job, the employee is regularly required to sit, talk, hear, and use
hands to finger, handle, or feel. The employee frequently is required to reach with hands and arms.
The employee is occasionally required to stand, walk, stoop, kneel, crouch, or crawl, and taste or
smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities
required by this job include close vision, distance vision, peripheral vision and depth perception.

While performing the duties of this job, the employee is occasionally exposed to toxic or caustic
chemicals and outside weather conditions. The noise level in the work environment is usually
moderate.
