I, Arantxa Stefi L. Santos, Filipino, of legal age, a resident of DCA 2 Apts, 2nd St. Mendiola,
Manila, after having been duly sworn in accordance with law, hereby depose and say:
1. That I am a holder of a School ID card bearing serial number C-110046 issued by San
Beda College.
2. That I used to pot said identification card in my bag.
3. That sometime on February 4, 2015, when I arrived home from school, I discovered my
School ID card was no longer in my bag.
4. That diligent search and efforts to locate said document, I could not find the said
documents.
5. That I am executing this affidavit to attest to the truth of the foregoing facts and to
request for a replacement of said School ID card.

IN WITNESS WHEREOF, I have here unto affixed my signature this 4th day of February 2015 at
