Varicocele
Eight things you need to know about varicocele:
1-Varicocele that can be defined as varicosis in testicular veins is the most common and
the most successfully treated cause of male infertility. It is scientifically proved that the
most successful varicocele treatment is the microsurgical method. The spermiogram
values of more than 80% of my 4,000 patients with microsurgical varicocele
operation increased in the last 22 years, pregnancy was provided in most of them and
negative trend terminated in 20%.
2-Microsurgical varicocele operation is not a simple operation. It is successful only in the
hands of experienced surgeons with special microsurgical training; otherwise, the
operation success rate is even below the rate of success of the old type surgery. The
failed applications of microsurgery can lead to very serious complications, or even the
loss of the organ. The failure rate in the old-style operation is 40%, failure rate in
microsurgery in the hands of an inexperienced surgeon is 50%, and the repetition rate if
done by an experienced microsurgery specialist is under 1%.

3-Varicocele begins at puberty and its negative effects increase over the years by the
time, which can lead to a- infertility, b-pain, c- a decrease in testosterone. It is possible
to ensure improvement in all of these problems with successful microsurgery. An
experienced expert should diagnose varicocele not with ultrasound but with a manual
inspection. Unnecessary operations are performed in many people diagnosed
incorrectly with varicocele under Doppler ultrasonography. The varicocele undetectable
in manual inspection should not be operated; it does not provide any good. Varicocele is
not an emergency and does not require emergency surgery; thus, it can be operated
within 1-2 months.

4-If a male patient has varicocele, assisted reproduction methods such as IVF, ICSI, and
vaccination should not be recommended. Varicocele reduces the strength of the
insemination of spouse's egg by impairing sperm chromosomal structure (DNA
fragmentation); the probability of miscarriage is high even if fertilization occurs.
Pregnancy can be achieved as a result of the operation for varicocele in patients who
failed after trying IVF so many times. In vitro fertilization should be considered as a last
resort; it raises congenital anomaly and disorder risks up to 30%, there are serious risks
of early menopause and much more for women. Microsurgery should be the first
treatment in case of varicocele.
5-The most successful method from a technical point of view for Varicocele with the
lowest complication risk is microsurgery applied by trained and experienced surgeons.
In the last 20 years, I did as nearly 4000 microsurgical varicocele operation; the rate of
technical achievement is over 99% in my line and spreading centers in the world, the

rate of serious complications reduces to 0% and insignificant complication rate to 34%. The probability of varicocele after surgery methods other than microsurgery is up to
40%, risk of hydrocele (swelling of testicle due to fluid accumulation between its
membranes) rises up to 40%, and arterial damage to 5%. Embolization as a radiological
method to treat varicocele should not be preferred because it has low success rate and
high rates of serious complications. Varicocele cannot be cured with medication or
herbal remedies.
6-Varicocele may not disappear because of inadequate surgery, can continue; It has an
incorrect expression as varicocele repeated. If varicocele is spotted with postoperative
examination, microsurgical correction surgery should be applied. Recently, because of
the unsuccessful applications of microsurgery, I have to do increasing proportion of
corrective microsurgery surgery. A portion of these patients who faced with serious
problems such as hydrocele or testicular atrophy after failed varicocele operations with
microsurgery or other old techniques, and had deteriorated spermiogram may benefit
from corrective microsurgery but not all of them.
7- Varicocele can be a cause of pain. If pain is pain due varicocele, it may disappear
after implementation of operation methods for pain. Varicocele may cause reduction of
the male hormone over time; this condition can be prevented with microsurgical
varicocele operation. Varicocele may cause testis to remain small in teenage boy,
microsurgical varicocele operation should be done without waiting.

Varicocele Surgery
To-do list before the arrival of the hospital for microsurgical
varicocele surgery:
1-Be sure to bring your varicocele-related test results all the way to the hospital such as
spermiogram, hormones.

2- If you want to clear the surgery region before, shave 10 cm of the groin area upward
from the root of the penis with both sides completely clean. Staff at the hospital can also
do this process when you are laid.
3- You must be hungry for at least 6 hours before the surgery; this hunger should be like
fasting in the form of not eating anything or even drinking.
4- Should not use anticoagulant drugs such as Aspirin in the previous week before the
surgery. Consult your doctor if you have a concern about this.
5- Bring with you a-tracksuit type pants elastic waistband, b-2 tight slip underwear.
6- Please inform in advance if you prefer to use a credit card. Your payment is a
package price for one-year period encompassing all including examinations, meals,
escort fees, anesthesia, medications, surgery, post-operative follow-up and fourth month
checkup. You do not have to consult accountancy in checkout. VAT is included in
charges.

What You Need To Know Upon Arriving To The Hospital For


Microsurgical Varicocele Operation:
1-Your doctor will visit you before and after the operation.
2- Do not eat or drink, as you would expect surgery and rest calmly.
3- Your doctor will visit you upon completion of the operation.
4- The contamination of your dressing is completely normal and a condition that we
have all been waiting for, do not worry.
5- Serum will remain attached until the evening hours and will be issued after the
application of antibiotics in the evening. You can get up to go to the bathroom before
going to bed by taking support from personnel or relatives and eat juicy food. It is highly
recommended that you do not stand up alone and eat other than the foods given on the
first day because of the possibility of dizziness depending on the anesthesia.

6- You can easily walk and make your breakfast next morning. In the same day, the
doctor or his assistant will make your dressing to get ready for checkout.
7-Start using your prescription drugs at once. You can use Zitromax 500 mg. for three
days as an antibiotic and Majezik tablet not more than twice a day as an analgesic for a
couple of days after surgery. It will be comfortable to wear a tight slip
underwear during the first week after the surgery, then you should not choose the slip,
we recommend that you wear a larger underwear. Your doctor will inform you about time
and place of the checkup after surgery.
8- Avoid the sutures from water for a period of one week after the operation; you can
clean other parts with water. The sutures will not be removed as they will be removed in
15-20 days by itself and after 15 days, you can help the removal of the sutures by using
washcloth. You can take a shower for a week after surgery, and have a sexual
intercourse observantly. Any movement or heavy lifting after surgery does not bring
harm to the success of your surgery but can just cause pain for the first 15 days or one
month. After one month, you can start doing sports in a controlled manner.
9- Please make your spermiogram and FSH tests (and DFI if done before surgery) in
four months after the surgery in the laboratory where your doctor made sperm analysis
and make an appointment for a free checkup.
