That I am the only surviving heir of GLORIA L. BARROGA who died single and
intestate at Candon City, Ilocos Sur on July 22, 2010.
That being the legal heir I know of the fact that her registered landholdings in the City of
Candon are as follows: Cornland with TD. No. 028-00366, Riceland with TD No. 029-00462,
Residential with TD No. 029-00668, Cornland/Residential with TD No. 029-00932;
That I know very well that she has no other landholdings in in the province of Ilocos Sur;
That the only property registered in my name including that of my spouse, is a residential
house with TD No. 029-00794, located at Patpata 2nd, Candon City;
That I am executing this affidavit in order to attest to the truth and correctness of the

WE, ALEJANDRO TOQUERO, CIPRIANO BARROGA, JR., ANTONIO FABRO, JR.,
and ROLANDO ROSARIO, all married, all of legal age, Filipinos, and residents of Barangay.
Patpata 2nd, Candon City, Ilocos Sur, after having been duly sworn to in accordance with law
hereby depose and say:
That we are members of the Barangay Council of Patpata 2nd, Candon City, Ilocos Sur;
That as Officials of our barangay we know very well the late MACARIO VALDEZ who
is the Cadastral claimant and the original owner of a tract of land, lot no. 3328 of the Cadastral
survey of Candon, Ilocos Sur;
That we also know of the fact that the after the death of late MACARIO VALDEZ his
legal heirs namely: JOHN VALDEZ-TOQUERO, ANSELMO VALDEZ-TOQUERO, RAFAELA
VALDEZ-SANCHEZ, and ROSA VALDEZ inherited the said property and occupied, cultivated
and possessed it since then..
That we, being residents of Patpata 2 nd, Candon City, we are acquainted with our
constituents. Since time immemorial, we do not know of a person named BENITA APALLA to
have been a resident of our barangay or an occupant and possessor of lot 3328 Cad 103. We are
not also aware of any surviving heirs and relative of said BENITA APALLA. We have been
informed that said BENITA APALLA had applied for a Free Patent over lot 3328 Cad 103.
That to the best of our knowledge, we know very well of the fact that said BENITA
APALLA was not the owner of lot 3328 Cad 103, Candon Cadastre;
That we are not related, either by consanguinity or affinity, to the Survey Claimant as
well as to the Declared Owners of the above-described parcel of land.
That we do not have any vested interest over lot 3328 , Candon Cadastre, which is subject
of the request for the cancellation of Free Patent Application and eventually for the issuance of
survey Authority that will lead to the verification and approval of a subdivision plan by the
DENR, LMS, R.O. I, San Fernando City, La union;
That we are executing this affidavit in order to attest to the truth and correctness of the
foregoing statements, and to justify and confirm the ownership of the heirs of MACARIO
VALDEZ, for whatever legal purpose it may best serve.

WE, ROSEMARIE GALDONES., and GEORGE MACARAEG both married, of legal
age, Filipinos, and residents of Barangay. Calaoa-an, Candon City, Ilocos Sur, after having been
duly sworn to in accordance with law hereby depose and say:
That we are members of the Barangay Council of Calaoa-an, Candon City, Ilocos Sur;
That as Officials of our barangay we know very well the late EUSTAQUIO GUZMAN
who is the Cadastral claimant and the original owner of a tract of land, lot no. 1688 of the
Cadastral survey of Candon, Ilocos Sur;
That we also know of the fact that the after the death of late EUSTAQUIO GUZMAN,
he was succeeded by his only son, LAUREANO GUZMAN who married Frisca Cortado.
LAUREANO GUZMAN and Frisca Cortado, were blessed with an only daughter, ELENA
GUZMAN who was also married to FRANCISCO CORTADO. At present the only surviving
heir of ELENA GUZMAN and FRANCISCO CORTADO is PRISCILA CORTADO, who
acquired all the rights and privileges over said property thru inheritance.
That being residents of Calaoa-an, Candon City, we are very much acquainted with our
constituents. That we know that PRISCILA CORTADO has continuously occupied, cultivated
and possessed the above-described property since the death of her parents up to the present;
That we are not related, either by consanguinity or affinity, to the Survey Claimant as
well as to the Declared Owner and her heir and successor-in-interest, of the above-described
parcel of land.
That we do not have any vested interest over lot 1688 , Candon Cadastre, which is subject
for the application of a free patent through the Department of Environment and Natural
Resources;
That we are executing this affidavit in order to attest to the truth and correctness of the
foregoing statements, and to justify and confirm the ownership of the heirs of EUSTAQUIO
GUZMAN, for whatever legal purpose it may best serve.

That we are the legal heirs of the late VICTORINO GACUTAN who died and intestate in
Candon, Ilocos Sur in the year________;
That the late Victorino Gacutan left a portion of a parcel of land (lot 2819, Cad 103
Candon Cadastre), situated at Patpata 1 st, Candon, Ilocos Sur, containing an area of 5,142 square
meters, declared in the names of VICTORINO GACUTAN and GERONIMO GACUTAN under
ARP No.____________;
That the said parcel of land was acquired by our father, Victorino Gacutan and his brother
Geronimo Gacutan on February 7, 1962 by virtue of a Deed of Absolute Sale, with docket no. 5;
on page no. 6; book no. III; series of 1962; duly notarized by Notary Public, Atty. Joselino S.
Abaya ( copy hereto attached);
That being the legal heirs of the late Victorino Gacutan, we are claiming the share of our
father which is equivalent to the one sixth (1/6) of the total area of the aforementioned property or
857 square meters;
That we do not agree to any subdivision scheme or plan whatsoever that does not
conform to our Claim on the above-described property as mentioned in the preceding statement;

I, ALEJANDRA VALDES, widow, of legal age, Filipino, and resident of Patpata 1st,
Candon City, Ilocos Sur, after having been duly sworn to in accordance with law hereby depose
and say:
That I am the registered owner of lot 3125 Cad 103 Candon Cadastre, situated at Brgy.
Ayudante, Candon, Ilocos Sur; containing an area of 5,428 square meters. Covered and embraced
by Original Certificate of Tiltle No. P-450;
That sometime in the month of July 2001, our dilapidated house was demolished due to
its damage and destruction brought about by Typhoon FERIA which hit our locality.
That during the demolition people who helped us found out that paper boxes in my room
were wet and others were torn down and eventually thrown to the waste pit;
That without my knowledge, the people who conducted the demolition have also thrown
the paper box containing the sepia film and blueprint copies of the survey plan, Psd-01-045353
and other lot documents;
That after the demolition me and my companions in the house exerted all our efforts to
look and search for the lost sepia film and blueprint copies of the aforementioned survey plan, but
to no avail we have not found any of them up to the present;
That I firmly believe that the documents stated in the preceding statements were thrown
in the waste pit during the demolition of our house and were burned thereafter;
That I am executing this affidavit in order to attest to the truth and correctness of the
foregoing statements and to request the DENR, Land management Sector, Region I, to issue a
copy of the sepia film of the said survey plan in lieu of the lost one.

qualified buyers.
