Full Administrator Vetting

To the special investigator completing this form - read this introduction in full:
The below is a screening of a subject who is being considered for an administrative staff position, including watchdog, or who is
already in an administrative position, and being considered for promotion.
The name of the subject must be edited into the document header. Make sure that you delete all instructions that are in
orange bold text at the bottom of most pages when you are done.
This form must be completed prior to the hiring of an administrative staff member, or alternatively, with the permission of
the division leader in charge, within seven days following hire. These two types of hire process are known respectively
as pre-hire and post-hire and this should be noted in the type of screening section.
Part D should be omitted (deleted) from the document if the subject is not being considered for a senior position.
Each section or subsection except those in Part E must start on a new page. Information of note should be highlighted
by filling the cell concerned with color, and not by using the highlight tool. Cells should be filled with yellow for suspect,
green for good, and red for highly suspect or serious misbehavior. To fill a cell, right click -> Properties -> Borders and
Shading -> Shading -> Fill and apply to that cell only. If a cell is red or black, the text inside should be white, and black
at all other times.
The classification level of all screenings is 4 unless otherwise instructed by the Director of Security, his Assistant
Director SIU, the Inspector-General or an Executive Administrator.
Ensure you complete the declaration on the last page. This is very important.
It is an offence under the Security Policy to discuss or otherwise disseminate information contained in a classification level to a
person or persons outside of that classification level. Any unauthorized sharing of information will result in immediate
suspension followed by termination and an indefinite community ban.
Remember to read all instructions and complete the declaration on the final page.
Final Reviewer (ADOS-SIU or Director): Ensure that this text and warning, as well as all other
instructional text is deleted from the document before converting it to PDF and sending it to the
security checks forwarder. Ensure that each section starts at the top of a new page.
