The MAST Questionnaire

The MAST Questionnaire addresses in detail a variety of medical and psychosocial disorders
that are regularly associated with substance abuse, as well as some specific questions about the
medical and legal consequences of alcohol abuse. Review it as a useful guide to further inquiry.

the night before and found that you could not remember part of
the evening?

Have you ever had DTs (delirium tremens), severe shaking,
heard voices or seen things that werent there after heavy
drinking?
Have you ever gone to anyone fore help about your drinking?
Have you ever been in a hospital because of drinking?
Have you ever been a patient in a psychiatric hospital or on a
psychiatric ward of a general hospital where drinking was part
of the problem?
Have you ever been seen at a psychiatric or mental health
clinic, or gone to a doctor or clergyman for help with an
emotional problem in which drinking has played a part?
Have you ever been arrested, even for a few hours, because of
drunken behavior?
Have you ever been arrested for drunk driving or driving after
