AFFIDAVIT OF TWO DISINTERESTED PERSON
We, PALSI BUSETAN and CARLOS TANGGACAN, both of legal
age, both married, Filipinos citizens and residents of Lagawa, Bauko, Mt.
Province, Philippines, after having been duly sworn to in accordance with
law hereby depose and state:
1 That we know personally know TEODORO CATIYAN PALOME,
the son of WANDEN PALOME and GANAY CATIYAN and born at
Lagawa, Bauko, Mt. Province, such facts of birth were duly
registered, and a member of the Social Security System (SSS);
2 That when he first applied for SSS, the name of his WANDEN
PALOME and GANAY CATIYAN, and his Birth Place is Lagawa, Mt.
Province. However, when he applied for another SSS, the name of
his parents are, CRISPIN MANGGES and AGNES MANGGES, and
his Birth Place is Banao, Bauko;
3 That the real name of Teodoros father is WANDEN PALOME, the
name Crispin Mangess is the alias name of Wanden Palome,
likewise the real name of Teodoros mother is GANAY CATIYAN,
and the name Agnes Mangges is the alias name of Ganay Catiyan,
which names is known to us, to his relatives, friends and in the
community. We, the Igorot particularly in the early days are keen of
calling one person in their Alias Name. Furthermore, his real birth
place is Lagawa, Bauko, Mt. Province;
4 The aforementioned errors/discrepancies regarding the names of
his parents and birth place were inadvertently written and not due
to any reason such as to evade in liability or whatsoever, hence, the
name WANDEN PALOME and CRISPIN MANGGES, pertain to
one and the same person, referring to the father of Teodoro
Catiyan Palome. Likewise, the name GANAY CATIYAN and
AGNES MANGGES, pertains to one and the same person,
referring to the mother of Teodoro;
5 That we know the above facts and circumstances because we are
long time family
friends and neighbors of the above
named person and her family;
6 That we are not in any way related to the above-mentioned person
neither by consanguinity nor affinity;
7 That we are executing this Affidavit to attest to the truth of all the
foregoing statement and to state the aforementioned facts and for
all legal intents and purposes that it may serve.
IN WITNESS WHEREOF, We have hereunto set our hands this 16 th
