FORMAL OFFER:
This Judicial Affidavit/Testimony of ANNE KUTIS is being
offered for the purpose of proving that Anne Kutis was one of the
witnesses

in

the

marriage

ceremony

conducted

between

FACUNDO JOSE HERMOSO AND KRISTINE SOTTO-HERMOSO.

I, ANNE KUTIS, of legal age, Filipino, single, with residence at
NO. 133 B DON MANUEL COR. APO BRGY LOURDES
QUEZON CITY, state under oath as follows:
PRELIMINARY STATEMENT
The person examining me is ATTY. JENNILYN V. TUGELIDA
with address at 17TH FLOOR OMM CITRA BLDG., SAN MIGUEL
AVE., ORTIGAS CENTER, PASIG CITY. The examination is being
held at the same address. I am answering his questions fully

conscious that I do so under oath and may face criminal liability

for false testimony and/or perjury.
Question (Q) 1:

Please state your name and other personal

circumstances for the record.

Answer (A) 1: I am Anne Kutis, 45 years of age, Filipino, single,
with residence at No. 133 B Don Manuel Cor. Apo Brgy
Lourdes Quezon City. I am a bank manager at BPI branch
Makati City.
Q2: Are you the same Anne Kutis, the witness in the marriage
between Facundo Jose Hermoso and Kristine Sotto-Hermoso?
A2: Yes, Ma'am.
Q3: How are you related to the contracting parties?
A3: I am the best friend of Kristine.
Q4: Is that Kristine that you are referring to the same as Kristine
Sotto-Hermoso?
A4: Yes, Maam
Q5: Do you know a certain Facundo Jose Hermoso?
A5: Yes, Maam. He is the husband of Kristine.
Q6: How did you meet him?
A6:

He was introduced to me by my best friend, Kristine, as her

boyfriend sometime in 1998.

Q7: Is that the first and last meeting that you had with Facundo
Jose?
A7: No, Maam.

Q8: When were the other meetings?

A8: I met him during the celebration of Kristines birthday and
other party were Kristine and I attended and during their
marriage.
Q9: So, you are present during their marriage?
A9: Yes, Maam.
Q10:

And when was that marriage contracted?

A10:The marriage was contracted on June 3, 2000.

Q11:

Where was it contracted?

A11:At the Court of Judge Martin Buaya, in Makati City.

Q12:

Why are you at the wedding?

A12: Because I am one of their primary sponsors at the wedding.

(Attached herewith as ANNEX A is a copy of the invitation used
during Facundo Jose and Kristines marriage)
Q13: What happened during the wedding?
A13: They exchanged I Dos before Judge Buaya and after the
whole wedding ceremony I was asked to sign the marriage
contract together with Facundo Jose, Kristine and Angel
Manzano.
I am executing this judicial affidavit to attest to the truth of the
foregoing and to constitute as direct examination as a witness in
this case.
I say nothing further.
