Important Advisory

ON THE DAY OF THE EXAMINATION, the Examinee MUST bring the following documents and materials:
1. NMAT Identification Form (To be submitted to the Examiner. Please read the guidelines below)
1.1. with signature of School Head if Examinee is a graduating college student, or
1.2. with photocopy of Transcript of Records (TOR) if Examinee is already a college graduate (or Certified True Copy
of Grades if Examinee is a new college graduate and TOR is not yet available as of test date).
2. One (1) valid Identification Document (I.D.) [To be presented to the Examiner for validation]
2.1. A valid I.D. must be original and bears the following features: (1) the Examinees name that exactly matches with
the name in the NMAT Identification Form, (2) a photograph that matches the Examinees face, and (3) the
Examinees signature.
2.2. Some examples of a Valid ID that may be presented, subject for verification, are official School I.D., Drivers
License, Passport, PRC I.D., NBI Clearance, Police Clearance, etc.
3. Examination Permit (To be downloaded from the NMAT website two weeks before test date. This serves as the
Examinees proof of payment of test fees and his/her notice of building and room assignment.)
4. Pencils (No. 2 or HB) and/with Erasers

Guidelines on the Submission of the Required Document(s)


The documents to be submitted on the test date shall depend on the educational status of the Examinee (i.e., Graduate or
Graduating) at the time he/she registered for the NMAT. The following document(s) MUST be submitted and will be collected by the
Examiner in the afternoon of the test date:
FOR GRADUATING STUDENTS
1.

NMAT Identification (ID) Form


A duly accomplished NMAT ID Form should bear the
Examinees signature and 2 x 2 colored picture with
complete name tag that matches with the name on the
NMAT ID Form. It must also have the school heads
signature on the designated space for signature of the
school head.

FOR GRADUATES

Transcript of Records
A photocopy of the Examinees Transcript of Records
(TOR) or Certified True Copy of Grades (if newly graduate
and TOR is still unavailable) should be submitted.

FOR EXAMINEES WHO MAY NOT BE ABLE TO COMPLY WITH THE DOCUMENTARY REQUIREMENTS ON THE TEST DATE
If you were unable to present a valid ID, your personal appearance and presentation of a valid ID is required at the designated CEM Test Center
office(s) where you took the test, whereas, document(s) duly accomplished NMAT ID Form and/or Transcript of Records, should be sent to the
NMAT Secretariat through a courier. The cutoff day for submission of documents is every Thursday. CEM will be updating the database of test
results every Friday so that you may view your online results on the following day or get your official NMAT Examinee Report Form on the following
working day.
Please be advised that your NMAT answer sheets will still be checked and scored together with all the other examinees. However, the release of
your test results (i.e., Online/Electronic Copy and Hard Copy/Official NMAT Examinee Report Form) will be withheld indefinitely until you comply
with the documentary requirements.
