AFFIDAVIT OF LEGITIMIATION
WE, ROMEL B. MENDOZA and HILDA PICARDAL MENDOZA, of
legal ages, now husband and wife, both Filipino citizens and of residents of P.
Burgos, Baguio City, after having been duly sworn to in accordance with law
state, that:
We had been living as husband and wife without the benefit of marriage
and born from us out of wedlock on 17 May 1999 is minor child named IVAHN
KHARL DUCUSIN PICARDAL in his certificate of live birth with our names
therein as his father and mother that have not married;
On 24 March 2000, we formalized our relationship in Baguio City when we
had our marriage solemnized by an authorized solemnizing officer in
accordance with the law;
At the time of the birth of our son IVAHN KHARL on 17 May 1999, there was
no impediment for any of us to get marry thus the execution of this AFFIDAVIT
OF LEGITIMATION to change the middle and family name of our son as
appearing in his Certificate of Live Birth so that his middle name should appear
as PICARDAL and that his family name should appear as MENDOZA which
middle name and family name he had been using since he attended school.
IN WITNESS WHEREOF, we hereunto set our hands this 11th day of February
2015 in Baguio City, Philippines to attest the truth of the foregoing recitals.
