CLASS POLICIES AND GRADING PROCEDURES
TESTS, QUIZZES, AND HOMEWORK
TESTS: There will be four written tests each worth 100 points. If you must miss a test,
you must notify the instructor PRIOR to the test. Failure to do so may result in not
being allowed to make up the test. Tests must be made up within ONE WEEK following
the original administration date. Make-up tests may not necessarily be the same test
given in class.

BEHAVIOR CONTRACT:
Activity 8 Behavior Change Contract found in the Behavior Change
Workbook (back of your textbook) (10 points): A problematic behavior will be
selected to change over the course of the semester. You will complete the contract
and commit to working on changing this behavior by the end of the semester. You
will track your behavior changes by using a health journal or spreadsheet which you
will turn in a copy of with your behavior change updates.

Behavior Contract Updates (5 pts each): Three assignments consist of writing

updates summarizing how you are progressing toward the goals set in your
behavior contract. These summaries must be typed or 1 point will be deducted.
(Please double space the update also.) Attach (paper clip or staple) a copy of your
health journal/spreadsheet detailing your daily/weekly progress to your update.
Two points will be deducted if this part is missing.
QUIZZES: (3 worth 20 pts each) Quiz 1 will cover the stages of change and the
steps in making a behavior change through a case study. Quiz 2 will cover
psychoactive drugs, tobacco and alcohol - Ch. 13. Quiz 3 will cover body composition
and weight management Ch. 6 & 9.
HOMEWORK: There are nine assignments each worth 5 points, four assignments
worth 10 points and one worth 15 points. Homework assignments turned in after the
due date will be subject to a 2 point penalty. Homework assignments more than one
week late will not be given any credit.

principles you want to live by. You can make it a top ten list or a personal mission
statement. Be creative! This must be typed unless you get prior approval from me.
VO2Max Lab (5 pts): The lab will be completed in class following the viewing of a
video of a student completing a VO 2 max test.
Body Composition Lab (10 pts): This lab consists of students having their
percent body fat assessed with skinfold calipers. For women the following sites will
be measured: tricep, suprailiac (side just above hip) and thigh. For men the
following sites will be measured: chest, abdominal and thigh. Please wear shorts
and a short sleeved shirt on this day to allow access to these sites which will make
the measurement more accurate. This lab will be completed in HS 1208. If you feel
uncomfortable with this lab, an alternative assignment may be completed. The
alternative assignment will consist of a three page typed scientific report describing
the importance of having healthy body composition and how body composition is
assessed. Proper referencing is required.
Your Diet vs. MyPlate (15pts): Record everything you eat and drink for one
average day of consumption. Complete Lab 8.1 by going to
www.ChooseMyPlate.gov using the SUPERTRACKER Food Tracker evaluation of your
