CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT is made and entered by and between:
The Provincial Government of Quirino with office address at Capitol Hills, Cabarroguis,
Quirino represented by GOVERNOR JUNIE E. CUA, herein after referred to as the FIRST
PARTY and BERNARD DULDULAO, GILBERT GUERRERO and BELMAR ALASKA,
of legal age, Filipino with residence address at Andres Bonifacio, Diffun, Quirino, herein
referred to as the SECOND PARTY.
WITNESSETH
That the PARTIES faithfully bind themselves to comply with the following:
1. TERM. This Contract of Services shall commence on November 26, 2014 and shall
expire on November 27, 2014unless otherwise terminated or extended by the parties in
writing.
2. POSITION. That the SECOND PARTY are hired as Contractual Surveyors;

4. RENUMERATION. That the FIRST PARTY shall pay the services of the SECOND
PARTY at the rates of EIGHT HUNDRED FIFTY PESOS AND THREE HUNDRED
FIFTY (Php800.00 & Php350.00) respectively per day.
5. DUTIES AND FUNCTIONS. That the SECOND PARTY agrees to perform survey
services for two (2) days.
OTHER OBLIGATIONS.

That both PARTIES agree to further observe the following

1. That the SECOND PARTY agrees to comply with the Contract.

2. That the SECOND PARTY shall submit a report of accomplishment and other
necessary documents as a result of such survey of said lots situated within the vicinity
of the Provincial Capitol.
3. That the FIRST PARTY reserves the right to terminate this contract at any time
during its lifetime and/or shall have repercussions or for unsatisfactory performance
when the office determines that there is no further need for the services contracted
for.
4. That the SECOND PARTY shall perform his assigned tasks at the designated place
of assignment or at such other places as maybe designated by the Provincial
Governor.

IN WITNESS WHEREOF, the PARTIES hereto have signed her/his Contract on the
20 day of November 2014, at Capitol Hills, Cabarroguis, Quirino.
