WAIVER, RELEASE AND HOLD HARMLESS AGREEMENT

This Waiver, Release and Hold Harmless Agreement (Agreement) is entered into by
and between __Laura Licari___________________ (Student) and John Bambenek on this
__26th_____ day of January, 2015.
WHEREAS, Student is enrolled in the CS 460 / ECE 419 Security Lab Course (course)
taught by John Bambenek at the University of Illinois; and
WHEREAS, Student understands and acknowledges that as part of the course he/she is
being given permission to attack computers and computer systems inside the course environment
during the duration of the course; and
WHEREAS, Student understands and acknowledges that he/she does not have permission
or authorization to access any other computer or computer system outside of the course
environment; and
WHEREAS, Student is authorized to enter into this Agreement; and
NOW, THEREFORE, in consideration of the permission granted by the University of
Illinois and John Bambenek, the sufficiency and receipt of which is hereby acknowledged, the
parties agree to the following terms:

Student acknowledges that he/she is participating in a computer security lab course at the
University of Illinois. Student acknowledges that as part of the course Student shall have access
to computer systems and shall be performing or participating in authorized attacks to computers
or computer systems. Student specifically acknowledges and understands that John Bambenek
and Student are authorized to perform these attacks only while engaged in the course and only
while using the computer systems located in Siebel Center Room 1129 during the scheduled
times and days of the course. Student acknowledges that he/she is only authorized to access and
attack computer systems in Siebel Center Room 1129 and, when off-campus, only attack IP
addresses 192.17.19.128-255 respectively during the duration of the course. Student further
understands and acknowledges that he/she may only access computers or computers systems and
perform attacks in accordance with the guidelines of the course and in accordance with the
permission granted in the course. Student understands and acknowledges that he/she is not
permitted to access or attack any other computer system or computer without prior lawful
authorization outside of the scheduled course. Student warrants that he/she shall not access any
other computer or computer system or perform any improper, unlawful or unauthorized actions
while a student in the course or at any other time. Student understands and acknowledges that
the knowledge gained from this course may not be used for any unlawful or unauthorized
purpose and affirms that he/she shall not use any knowledge gained from this course for any
unlawful or unauthorized purpose. Student shall not use any information gained from the course
to access accounts or assets outside Siebel Center Room 1129 or, when off campus, IP addresses
outside the 192.17.19.128-255 range. Attacks using the IllinoisNet or UIUCnet Wireless
networks are not authorized. This authorization expires when the student withdraws from the
class or on May 16, 2015.

Student and John Bambenek hereby mutually agree to waive any and all claims against
each other, now existing or existing in the future, under any theory of recovery, related in any
way to this computer security lab course or the knowledge and experience gained from this
course which arise during or result from Students participating in this course.
3.
Student warrants and agrees that he/she shall not perform any unlawful, improper, or
unauthorized actions while enrolled in the course and following the course. Student warrants
that he/she has carefully read and reviewed this Agreement and fully understands the terms and
conditions contained herein.
