having been duly sworn to in accordance with law, do hereby depose and say:
That we are the parents of RAYMUND PAJARILLO ROSALES, minor, born on FEBRUARY 26,
1997 and presently residing at BLK 68,LOT 15,ONYX ST. DECA ESPERANZA TIGATTO DAVAO CITY,
Philippines;
That we are giving our full consent for our son to travel with European Vessel as APPRENTICE on
his own, but accompanied by and will full custody of DMMA COLLEGE OF SOUTHER PHILIPPINES in
collaboration with the DBP Sponsorship Program who will take full responsibility for all his expenses,
subsistence and accommodation throughout the duration of his travel and ensure that he will not become a
public charge while in travel and
That we are executing this affidavit for the purpose of applying for travel clearance for our son with
the Department of Social Welfare and Development (DSWD).
