I, Pubbisetty Vijaya Lakshmi W/o.Late Venkateswarlu, aged
49 years, Hindu, resident of near Saibaba Temple, Kandukur Village
and Mandal, Prakasam District, do hereby solemnly affirm and sincerely state
as follows:I am the deponent herein and 3 rd petitioner in the main O.P.

The O.P. is

filed for compensation for the death of my Mother, who died in a motor
accident. Later the same was settled before the Lok-Adalat for an amount of
Rs.1,00,000/- on 06-12-2014 against the 3rd respondents.

The 3rd respondent

deposited an amount of Rs.1,00,000/- only into the Court. The Honble Court
was awarded an amount of Rs.20,000/- towards my share of compensation
amount. The Honble Court was pleased to permit me to withdraw my entire
compensation amount as per decree direction.

I am entitled to withdraw the

same and there are no attachments or stay or appeal on the said amount. I,
under take that if the Honble Court directs me to re-deposit the amount, I will
deposit the same. I am herewith filing my Bank Pass-Book (Xerox).
Hence I pray that this Honble Court may be pleased to issue a cheque for
Rs.20,000/- (Rupees Twenty thousand Only) in favour of the Branch Manager,
HDFC Bank Ltd., Kandukur Branch, Prakasam District, and he may be directed
her to credit the same to the S.B.A/c No.50100032153882 of the petitioner in
the interests of justice.

DEPONENT
Solemnly affirmed and signed before me the contents of the affidavit is read
over and explained in Telugu to the deponent and admitted her to be correct on
