1. At about 7:15 PM of March 31, 2014, a telephone call received by this office
from the staff of San Juan District Hospital, informing that there was a victim
of traffic accident being treated thereat. In response duty investigators PO3
Jeremy M. Ambita and PO2 Christopher R. Escabel proceeded to the said
hospital, purposely to conduct investigation. Initial investigation found out
that on or about 6:55 Pm of March 31, 2014 along Provincial Road of Brgy.
Laiya Ibabao, San Juan, Batangas, one motorcycle (for registration) Honda
Supremo, colored red, owned by Mrs. Marilyn Mojares Santero of Brgy. Purok
Siete (7), Emerald, Pangao, Lipa City and being driven by Leo Adante y
Rivarez, 30 years old, married a holder of Professional Drivers License with
number DO1-03-198688 valid until 2015-02-02 and a resident of Brgy. San
Teodoro, Bauan, Batangas while travelling heading north direction a dog
coming from the right portion of the road suddenly appeared, thereby
causing it to accidentally hit and bumped to the electrical post of BATELEC II
placed on the left side of the road. Said motorcycle incurred undetermined
amount of damages, while the driver and the back rider identified as Alvin
Sunga y Arceo, 40 years old, married and resident of Brgy. Pangao, LIpa City
both suffered injuries on the different parts of their bodies and later transfer
to Banay-Banay Hospital, San Jose, Batangas for further treatment and
confinement.

1. At about 12:15 PM of July 20, 2014, Mr. Noel Tolentino y Antolin. 41
