ORGANIZING

A. Chain of Command
The Infirmary Services Unit was headed by the university infirmary unit head physician.
Under him are head of the out-patient department, the University Dentist, and the University
Head Nurse / Nurse II. The command is coming from the University head physician down to
his personnel.

B. Staffing
Recruitment
According to the head physician, hiring of their new employees will depend on their
needs in manpower. The physician will be the one to request for a new employee, and once it
was approved in the administration office they will post an advertisement for a month. After a
month they will select from those who applied for the job. The head physician will join the
selection committee during the interview of the applicants and in deciding whom to hire from
those applicants. The consultants shall be chosen based on their qualifications and field of
specialization.
The following shall be used as minimum requirements in choosing the core of medical
consultants:
a. Should have been a medical practitioner for atleast three years.
b. With medical certificte from accredited hospital with Residency Training Program
concerning his field of specialization.
c. No pending administrative / civil case.
d. Unquestionable dedication towork as certified by his former/ present employer.
e. Follow the ethical standards of medical practitioner.
f. Willing to share his expertise by actively conducting or participating in medical related
activities.
g. Willing to teach in the College of Nursing on a part-time basis as needed.
h. Willing to bring his equipment for use in the OPD of the CvSU Infirmary.
Based also on our interview with the head, most of their personnels are contractual. But,
the decision is in the administration office and on him on how long will a personnel will stay on

a certain position.

C. Delegation
Based on our intterview, the University Infirmary unit head physician is the one who is
responsible in delegations of task. He delegated the works based on his personnel's
qualifications and area of specialization.

D. Responsibility, Authority and Accountability


I. University Infirmary Unit Head Physician
1. Carry out the objectives of the CvSU Health Service through efficient of the following
functions:
a. Maintain the proper general health of the CvSU populace through primary promotion
and secondary care of the constituents, comprising the studentry, its personnel and dependents.
b. Promote the general health through health education.
c. Apply preventive and therapeutic measures and inoculation for the prevention of
epidemic and spread of communicable diseases through coordination with the Department of
Health and other health agencies.
d. Formulate administrative orders, health policies and procedures with the
Administrative Council.
2. Provide or perform medical consultation, treatment and attend to emergency cases of
ailing students and personne of CvSU.
3. Conduct periodic medical and physical examination of students and personnel health
record.
4. Oversee the maintenance of evaluation of students and personnel that require
hospitalization.
6. Responsible for thedispatch of ambulance.
5. Make arrangement on referral cases of medico-legal in nature to security office;
7. Responsible for the requisition and safe keeping of medicine,medical supplies /
materials and equipment.

8. Act as a liaison between the administrations or within and outside agencies.


9. Represent the CvSU Infirmary Health Services in official and social functions.
10. carry out personnel management and development.
II. Head Nurse / Nurse II
1. Responsible for the overall supervision of the medical ancillary services.
2. Schedule upon approval of the Unit head physician the duties / assignment of all
personnel under her supervision.
3. Coordinate with the unit head physician in the implementation of the rules and
regulation and maintenance of medical ancillary services.
4. Assist the Unit head physician in the preparation of records and report any misconduct
of the personnel under supervision.
5. Submit monthly report of accomplishment.
6. Conduct orientation / seminar session of personnel under her supervision.
7. Assist physician in management of serious and difficult emergency cases.
8. Assist physician in conducting periodic medical and physical examination of students
and CvSU personnel.
9. see to it that all medicines, medical supplies and materials in the infirmary are readily
available at all times.
10. Oversee the maintenance and proper arrangement of students and personnel health
records.
11. Perform other duties assigned by the Unit head physician from time totime.
III. Staff Nurse
1. Apply the nursing process in:
a. Identifying nursing needs or problem of the clients through clinical observation,
interaction and physical assessment.
b. Planning and implementing the nursing intervention.
c. Evaluating the effects of nursing intervention and evaluation.
2. Assist the head nurse and the physician in providing first aid treatment to emergency

cases and treating minor injuries and ailments.


3. Assist physician in condcting periodic medical and physical examination for students
and CvSU personnel.
4. Be responsible in sterilizing of medical instrument after using it.
5. Oversee the cleanliness of the infirmary building.
6. Oversee the maintenance and cleanliness of medical instruments and equipment.
7. Oversee the maintenance and evaluation of students and personnel health records.
8. Submit monthly report of accomplishment to head nurse and to Unit head physician.
9. Be available for duty as exigency arises.
10. Perform other duties, which the Nurse II which serves as the head nurse, and the
physician orders for the good of the services.
IV. University Dentist
1. Attend to the dental care / need of the students, faculty and employees.
2. Perform annual dental examination of students, faculty and employees.
3. Record complaints, diagnosis, and treatment of all dental patients.
4. Attend to the upkeep and maintenance of the dental clinic, equipment, instrument and
other facilities.
5. Prepare requisition of supplies, equipment and other materials for the efficient function
of the clinic.
6. attend convention, seminar workshop, refresher course and meeting to improve his/ her
dental knowledge and development.
7. Perform other duties, which the Unit head physician orders for the good of the service.
V. Medical Technologist
1. Perform routine laboratory and special examinations as requested by the attending
physician upon presentation of official receipt payment.
2. Keep all laboratory result in a record book.
3. Submit laboratory examination report / result to the requesting physician.

4. Keep / maintained all the laboratory equipment, apparatus and instruments on


laboratory room clean at all time.
5. Be responsible for the renewal of license and permit to operate the laboratory.
6. Submit monthly accomplishment report.
7. Perform other duties as prescribed by the head nurse and by physicians.
VI. Radiology Technician
1. Take X-ray of patients upon order of attending physician after presentation of official
receipt payment.
2. Maintain the proper identification of X-ray plates examined.
3. Prepare proper preparations prior to X-ray examinations.
4. Develop the exposed X-ray film.
5. Assist the radiologist during film reading.
6. Do the typing, recording, and filling of patient data, X-ray results and films.
7. Be responsible for the requisition of X-ray films, supplies and materials.
8. See to it that the X-ray machine is in good condition.
9. Be responsible for the renewal of license and permit to operate of X-ray machine.
VII. Ambulance Driver
1. Always be ready and available at all times particularly during emergency.
2. Be on call anytime during day and evening.
3. Attend to the maintenance of the ambulance and other necessities to make it ready for
immediate use.
4. Perform other duties assigned by his superior like the following:
a. Perform special errand.
b. Keep and maintain trhe cleanliness of the Infirmary building and surrounding
premises.
c. Maintain the safety of the building and other infirmary properties.

E. Communication
According to the head physician he uses a democratic way of leading which means that
less control is maintained. He always conduct a meeting every month as a part of their activities
to know each other's concerns, to plan and involve his personnel in decision making. This
leadership style shows that the communication flows up and down as the personnel is directed
through suggestions and guidance.
F. Span of Control
The University head physician is the one supervising different section. His staffs are
limited and work according to their specialization. But if the workers are highly skilled, then
their numbers may be increased.
